磁共振弥散加权序列对直肠癌术前分期评估的价值

摘　　要：评价磁共振成像弥散加权序列在直肠癌术前分期诊断价值。2007年1月至12月67例直肠癌患者进行术前MRI检查,对67例直肠癌患者分别采用b值为600、1000s/mm2,进行MR扩散加权序列检查,测定瘤体、淋巴结和皮下脂肪ADC值,并与病理结果作对照;比较不同b值下瘤体分化级别与ADC值有无差异,同时比较良恶性淋巴结ADC值有无差异,采用t检验和方差分析进行统计学分析。不同b值条件下,瘤体及淋巴结的ADC值差异有显著性意义(P<0.05),随着b值升高,组织ADC值降低。同一b值下瘤体分化级别组间无差异,良、恶性淋巴结ADC值组间有差异。磁共振扩散加权序列对直肠癌和淋巴结检出是一个可行性方法,ADC值对良恶性淋巴结鉴别有一定价值,DWI序列可以作为常规MR扫描序列。To assess the value of diffusion - weighted imaging sequence in the preoperative staging of rectal cancer. Between January and December 2007, sixty - seven patients with operable rectal cancer underwent preoperative MR imaging with single - shot echo plannar imaging（EPI） diffusion - weighted sequences （b = 600, 1000s/mm^2）. All of them were compared with pathologic staging. The apparent diffusion coefficients （ADCs） of the tumors、lymph nodes and lip in each patient were measured to evaluate the correlation between ADC and pathological classification of each tumor , and between benign and malignant lymph nodes. The mean ADC values of tumor and lymph nodes were decreases following the ascending of b values . No overt correlation can be pointed out between ADC and pathological classification of each tumor, and there were correlation between benign and malignant lymph nodes of the mean ADC values. Diffusion - weighted imaging is a rapid and feasible method in detecting rectal cancer and lymph nodes, the mean ADC values give help for distinguishing between benign and malignant lymph nodes, DWI may be regarded as a conventional sequence in the preoperative staging of rectal cancer.
