34例腮腺多原发性肿瘤临床分析

摘　　要：目的探讨腮腺多原发肿瘤临床特点、病理类型、治疗方法及生存现状。方法回顾分析34例腮腺多原发肿瘤临床资料，分析肿瘤病理类型、患者性别、年龄、生活习惯、治疗方案等。结果腮腺多原发肿瘤中沃辛瘤占79％（27例），多形性腺瘤占24％（8例）。单侧22例，双侧12例，同时性肿瘤28例，异时性肿瘤6例，男女比例为2．4：1，年龄25～81岁，平均年龄58岁，27例嗜好吸烟。85％采用保留面神经的瘤体及腮腺浅叶切除术。结论腮腺多原发性肿瘤大多数为沃辛瘤，其次为多形性腺瘤，男性好发，吸烟是该病重要致病因素之一，保留面神经的瘤体及腮腺浅叶切除术是一种较好的治疗方式。Objective To study the clinical characteristics, the pathological types, the treatment and life status of the multiple primary tumors in parotid gland. Methods 34 case of the multiple primary tumors in parotid gland were retrospectively analyzed. The data included the types of pathology, sex, age, smoke and drinking history, treatment et al. Results Most of the multiple primary tumors in parotid gland are Warthin＇ s tumor accounting for 79 percent （27 case）, the following are pleomorphic adenoma accounting for 24 percent （8 case）, 22 patients are unilateral parotid, 12 patients are bilateral parotid, the tumors are found and diagnosed synchronously in 28 patients, and metachronously in 6 patients ; the male： female ratio of the multiple primary tumors is 2.4： 1. The median age is 58 years （range 24 -80 years） ; 27 patients are heavy smokers, 85% of all patients are taken lateral parotidectomy. Conclusion Most of the multiple primary tumors in parotid gland are Warthin＇ s tumor, the following are pleomorphic adenoma; the multiple primary tumors are more common in men; smoking is a very important malgenic factor; lateral parotidectomy is an optimal treatment of these tumors.
