胃癌螺旋CT征象与CD44v6及MMP-9表达的关系

摘　　要：目的探讨胃癌螺旋CT征象与CD44v6及MMP-9表达的关系。方法对60例进展期胃癌患者于手术前1周内行SCT平扫及三期动态增强扫描,术后应用免疫组织化学染色的方法测定肿瘤组织CD44v6和MMP-9蛋白的表达。结果①60例进展期胃癌组织中CD44v6和MMP-9的阳性表达率分别为60%(36/60)和58.3%(35/60)。②SCT图像显示的肿瘤强化幅度≥30HU、<30HU组的CD44v6阳性表达率分别为76.5%(26/34)和38.5%(10/26),MMP-9阳性表达率分别为64.7%(22/34)和50.0%(13/26))。③浆膜侵犯有、无组CD44v6阳性表达率分别为72.2%(26/36)和41.7%(10/24),MMP-9阳性表达率分别为75%(27/36)和33.3%(8/24)。④淋巴结转移阳、阴性组CD44v6阳性表达率分别为68.2%(30/44)和37.5%(6/16),MMP-9阳性表达率分别为68.2%(30/44)和31.3%(5/16)。结论CD44v6的表达与SCT显示的肿瘤强化幅度、浸润深度、淋巴结转移有关,MMP-9的表达与SCT显示的浸润深度以及淋巴结转移有关。Objective To investigate the relationship between spiral CT features and expressions of CD44v6 and MMP-9 in advanced gastric carcinoma.Methods Both plain SCT scanning and three periods contrast-enchanced scanning were performed in 60 patients with advanced gastric cancer.All patients underwent gastrectomy within one week after SCT scanning.After operation,the specimens were stained by immunohistochemical staining to detect the expressions of CD44v6 and MMP-9.Results ①Of 60 cases advanced gastric carcinoma,the positive expression rate of CD44v6 and MMP-9 were 60%（36/60） and 58.3%（35/60） respectively.②The CD44v6-positive rates were 76.5%（26/34） and 38.5%（10/26）respectively in the contrast enhancement over and under 30 HU,the MMP-9-positive rates were 64.7%（22/34） and 50.0%（13/26） respectively in the contrast enhancement over and under 30 HU.③The CD44v6-positive rates were 72.2%（26/36） and 41.7%（10/24） in patients with and without serosal invasion respectively,the MMP-9-positive rates were 75%（27/36）and 33.3%（8/24） in patients with and without serosal invasion respectively.④The CD44v6-positive rates were 68.2%（30/44） and 37.5%（6/16）in patients with and without lymph node metastasis respectively,the MMP-9-positive rates were 68.2%（30/44） and 31.3%（5/16） in patients with and without lymph node metastasis respectively.Conclusion The expression of CD44v6 is closely correlated with the degree of contrast enchancement,depth of invasion and lymph node metastasis shown by SCT images.The expression of MMP-9 is closely correlated with lymph node metastasis and depth of invasion shown by SCT images.
