摘　　要：目的探讨肝细胞肝癌(HCC)发生、发展因素,为HCC诊治提供指导。方法应用免疫组织化学法检测60例HCC中Livin、survivin与胰岛素样生长因子-2(IGF-2)的表达,应用COX风险模型及Log-rank检验分析其与预后相关性。结果HCC中Livin、survivin的表达阳性率均高于癌旁组织及正常组织(P<0.05),且与肿瘤镜下门静脉癌栓发生密切相关(P<0.05);IGF-2表达与HCC肿瘤镜下门静脉癌栓发生、肿瘤病理分期以及肝硬化密切相关(P<0.05);相关性检验示Livin、survivin与IGF-2表达均有明显相关性,三者均为HCC的独立预后因素。结论IGF-2与Livin、survivin相互作用共同参与HCC的发展,确定其水平有助于判断HCC的预后并指导术后治疗。Objective To study the expression of Livin, survivin and IGF-2 in human hepatocellular carcinoma （HCC） and its clinical significance. Methods Expression of Livin, survivin and IGF-2 in 60 HCC patients were assessed by immunohistochemistry method. Twelve candidate factors associated with prognosis were analyzed by the COX proportional hazards model and Log-rank test. Kaplan-Meier was used to estimate the survival rate. Results The positive rate of Livin, survivin and IGF-2 expression were 32/60 （53.3%） ,39/60 （65.0%） ,31/60 （51.7%） respectively. The expression rate of Livin and survivin in HCC tissue was higher than that in the adjacent non-tumorous liver tissue and the normal liver tissue. There was no significant correlation between the expression of Livin and survivin in HCC and the tumor size, capsule status, cirrhosis,the classification of Child-Paugh ,TNM stage, except the portal vein tumor thrombus. There was no signifi- cant correlation between the expression of IGF-2 and capsule status, the classification of Child-Paugh and cirrhosis. However, the positive rate of IGF-2 was correlated significantly with portal vein tumor thromb,liver cirrhosis and TNM stage. There was a close correlation between the expression of Livin, survivin and IGF-2. The multivariae analysis showed that Livin, survivin and IGF-2 were independent significant prognostic factors of HCC. Conclusion The upexpression of Livin, survivin and IGF-2 has a close correlation and holding a prognostic significance in HCC.
