摘 要:目的探讨孕酮对SD大鼠实验性高眼压视网膜缺血-再灌注损伤后的保护作用及机制,并评估其在视网膜缺血性疾病中的应用价值,为临床应用提供实验依据。方法采用眼内灌注法,前房灌注生理盐水制成视网膜缺血-再灌注模型。32只SD大鼠随机分为对照组、正常组、二甲基亚砜溶剂对照组、孕酮治疗组。孕酮治疗组于缺血前30min与再灌注2h后按4mg/kg腹腔内注射孕酮溶液;二甲基亚砜溶剂对照组于缺血前30min和缺血后2h分别腹腔注射相当体积的二甲基亚砜。各组分别于再灌注6h后空气栓塞法处死动物。用硫代巴比妥法测定丙二醛(MDA)含量,黄嘌呤氧化酶法测定超氧化物歧化酶(SOD)活性,并观察视网膜组织形态学与超微结构的改变。结果(1)再灌注6h后,对照组视网膜MDA含量较阴性对照组显著增高(P<0.01),治疗组视网膜MDA含量均低于对照组(P<0.01);对照组视网膜SOD活性较阴性对照组显著降低(P<0.01),治疗组视网膜SOD活性均高于对照组(P<0.05)。(2)光镜下,对照组视网膜内核层、神经纤维层变薄,电镜下出现核固缩,线粒体空泡化,内质网肿胀。孕酮治疗组视网膜神经节细胞层、神经纤维层的形态学明显改善。结论SD大鼠视网膜缺血-再灌注损伤中,视网膜组织MDA含量升高,SOD活性下降,视网膜组织病理学显著改变。孕酮可降低视网膜组织MDA含量,提高SOD活性,对新生SD大鼠视网膜缺血-再灌注损伤起到保护作用。Objective To investigate the protective effect of progesterone against high intraocular pressure-induced ischemia-reperfusion (IR) injury. Methods Twenty-four SD rats were randomly divided into normal control, IR model, dimethyl sulfoxide (DMSO) solvent treatment group, and progesterone treatment group. In the latter 3 groups, retinal IR injury was. induced by intraocular injection of saline. In the progesterone group, intraperitoneal injections of 4mg/kg progesterone were administered 30 min before and 2 h after ischemia, and an equivalent volume of DMSO was used in the DMSO group. The content of malondialdehyde (MDA) and superoxide dismutase (SOD) activity were measured by speetrophotometer after the treatment, and the pathological changes of the retina were observed by transmission electron microscope and light microscope. Results Six hours after reperfusion, the content of MDA in the model group was significantly higher than that in the normal control group (P〈0.01), but lower than that in progesterone treatment group (P〈0.01); reverse changes in SOD activity was observed. In the model group, the inner nuclear layer and nerve fiber layer became thinner with obvious cellular pathologies including nuclear condensation, mitochondria vacuolization and endocytoplasmic reticulum swelling. Progesterone treatment markedly alleviated these pathologies in the inner nuclear layer and nerve fiber layer of the retina. Conclusion Progesterone offers protection of the retina against IR injury in SD rats by increasing SOD activity and decreasing MDA content in the retina.
