摘 要:目的通过检测HLA-I类抗原及CD8分子在不同宫颈病变组织中的表达,探讨HLA-I类抗原及CD8分子的表达与宫颈癌之间的关系。方法采用免疫组织化学SP法检测宫颈癌、宫颈上皮内瘤变及慢性宫颈炎组织中HLA-I类抗原及CD8分子的表达,并分析其与各临床病理指标的相关性。结果HLA-I类抗原在有核细胞中均有表达,其在宫颈癌、宫颈上皮内瘤变及慢性宫颈炎组织中的阳性表达率分别为22.6%、100.0%及100.0%,宫颈癌中的表达率明显降低(P<0.01),HLA-I类抗原在I期宫颈癌组织中的阳性表达率为46.7%明显高于II期0.0%(P<0.01);CD8分子在淋巴细胞中表达,其在宫颈癌、宫颈上皮内瘤变及慢性宫颈炎组织的阳性表达率与HLA-I类抗原相似,分别为22.6%、95.5%及100.0%,宫颈癌中的表达率明显降低(P<0.01),CD8分子在I期和II期宫颈癌组织中的阳性表达率分别为46.7%和0.0%(P<0.01)。HLA-I类抗原和CD8分子随着临床病理分期的升高而逐渐的表达减少甚至无表达。而二者与宫颈癌患者的组织分化程度和有无淋巴结转移无关(P>0.05)。HLA-I类抗原和CD8分子的表达存在正相关(Spearman秩相关系数rs=0.808,P<0.0005)。结论HLA-I类抗原和CD8分子在宫颈上皮内瘤变和宫颈癌组织中表达减少甚至无表达,二者在宫颈上皮内瘤变和宫颈癌的发生、发展中可能起着重要的保护作用。Objective To examine the expressions of HLA class Ⅰ antigen and CD8 in various cervical diseases and investigate their association with Cervical cancer. Methods The expressions of HLA class Ⅰ antigen and CD8 in cervical tissues sampled from patients with cervical cancer, cervical intraepithelial neoplasia (CIN), and chronic cervicitis were detected using SP try. The association of the expressions of HLA class Ⅰ antigen and CD8 with the clinicopathologic indices of the patients was analyzed. Results The positive expression rates of HLA class Ⅰ antigen in cervical cancer, CIN, and chronic cervicitis were 22.6%, 100.0%, and 100.0%, and the positive expression rates of CD8 were 22.6%, 95.5%, and 100.0%, respectively. The positive rates of HLA class I antigen and CD8 were significantly lower in patients with cervical cancer (P〈0.01). Patients with stage Ⅰ cervical cancer had significantly higher positive rates of HLA class Ⅰ antigen and CD8 than those with stage Ⅱ cervical cancer (46.7% vs 0.0%, 46.7% vs 0.0%, both P〈0.01). The expressions of HLA class Ⅰ antigen and CD8 decreased with the progression of the clinicopathological stages, and may even become undetectable. The expressions of HLA class Ⅰ antigen and CD8 were not related to the differentiation degree of the tumor or lymph node metastasis (P〉0.05). A positive correlation was found between HLA class Ⅰ antigen expression and CD8 expression. Conclusion The expressions of HLA class Ⅰ antigen and CD8 are down-regulated or deleted in CIN and cervical cancer, and they may play important roles in the development and progression of CIN and cervical cancer.
