爆裂性眶壁骨折的临床表现和CT所见

摘　　要：探讨爆裂性眶壁骨折的临床表现、ＣＴ所见和治疗手段，经ＣＴ确诊的爆裂性眶壁骨折１８例，其中内侧壁型１４例，下壁型２型，内侧合并下壁型２例。非手术治疗１３例，手术治疗５例。临床主要表现为眼球内陷、复视和眼球运动障碍。ＣＴ显示眶壁骨折移位，眶腔体积增大，眶组织嵌顿和副鼻窦损伤。非手术治疗１３例，全部临床治愈，手术治疗５例，３例临床治愈，２例好转。结论：爆裂性眶壁折引起眶壁骨组织破坏导致眼眶组织解剖异常，ＣＴ可以显示眶壁结构破坏和眶组织及副鼻窦损伤的变化。Abstract To study clinical and CT findings as well as management of blow out fractures.Among 18 case of blow out fractures diagnosed by CT (14 cases of medical fractures,2 cases of floor fractures and 2 cases both medical and floor fractures),13 cases were treated without surgery and 5 cases with surgery.The main clinical findings were enophthalmos,diploia and abnormal eye movements.CT findings were displacement of the orbital wall,enlargement of the orbital volume,soft tissure trauma and damage in the accessory sinuses of the nose.13 cases without surgery were all clinical cureed.Among 5 cases with surgery,3 cases were clinical cured and 2 cases got improvement.We conclude that indication of the surgery depends on the degrees of the fractures and the soft tissures damage,including the extraocular muscles and the important nerves in orbit.
