评价视野检查在外伤性视神经病变临床中的作用

摘　　要：目的客观评价Octopus 101视野计在外伤性视神经病变诊断和治疗过程中的指导性作用。方法对86例(86眼)诊断为外伤性视神经病变的临床病例进行Octopus 101视野计检查,分析其图形、MD、LV值,同时治疗第3 d、1周、2周、3周后连续视野检查确定治疗的效果及预后判断。结果外伤性视神经病变多为单眼受伤,多为单眼视野改变为其特点。其中视野完全缺损20例;视野阈值整体下降15例;局限性视野缺损31例,局限性视野缺损形状不规则,LV值均大于2db;仅存视岛20例。治疗后跟踪MD值下降2db判断为治疗有效。结论自动电脑视野计对外伤性视神经病变的诊断和疗效、预后判断具有指导作用,具有相对客观性。Objective To illustrate the value ofperimetry in diagnosis and prediction of patient with traumatic optic neuropathy. Methods The perimetry was performed on 86 patients ofdiagonised traumatic optic neuropathy . We exam MD and LV continuously after treatment in 3 days, 1 week, 2 weeks and 3 weeks. The results were analysed. Results The traumatic optic neuropathy was only one eyes, in the mean time the visual field defects was only one eye. Among 86 cases, the optic nevers were completely injutied in 20 cases, in which the visual fields were completely lost. The threshold of visual field of 15 cases decreased equably, in the rest 41 cases, part of the optic never were injuried, their visual fields were lost partly. The forms of visual field defects were irregulary. If MD decreased 2db after treatment , we can think the treatment is effective. Conclusion Considering the simple, safe and convenience of perimetry , as well as its accuracy of outcome in the cases of traumatic optic neuropathy, perimetry is helpful in the diagnosis, treatment of traumatic optic neuropathy.
