乳腺癌患者外周血中角蛋白19 mRNA,乳腺小粘蛋白mRNA和泌乳素诱导蛋白mRNA的检测及其临床意义

摘　　要：目的:研究乳腺癌患者外周血中角蛋白(CK)19mRNA,乳腺小粘蛋白(SBEM)mRNA和泌乳素诱导蛋白(PIP)mRNA的表达,并探讨三者在微转移检测中的意义。方法:用RT-PCR检测乳腺癌患者50例、结直肠癌20例、乳腺良性肿瘤21例、健康者20例外周血中的CK19mRNA、SBEM mRNA、PIP mRNA。结果:CK19mRNA在乳腺癌组的阳性率(36.0%)明显高于乳腺良性肿瘤组(0.0%,P<0.05)与健康者(0.0%,P<0.05)。SBEM mRNA、PIP mRNA在乳腺癌组的阳性率(28.0%、18.0%)明显高于其它3组(0.0%,P<0.05)。联合检测3个基因,可将阳性率提高到62.0%。结论:乳腺癌患者外周血中的SBEM mRNA、PIP mRNA均可能作为微转移标志物;而CK19mRNA作为乳腺癌微转移标志物的特异性尚待探讨;联合检测三者可提高乳腺癌外周血微转移的检出率。Objective： To investigate the expressions of cytokeratin（CK） 19 mRNA, small breast epithelial mucin （SBEM） mRNA and prolactin-inducible protein（PIP） mRNA in peripheral blood of patients with breast cancer and their clinical significance. Methods： The expressions of CK19 mRNA, SBEM mRNA and PIP rnRNA in peripheral blood of 50 patients with breast cancer, 21 patients with breast benign tumor, 20 patients with eolorectal cancer and 20 healthy volunteers were detected with reverse transeription-pelymerase chain reaction. Results： The positive rate of CK19 mRNA expression in the breast cancer group（36.0%）was notablely higher than both breast benign tuner group（0.0%, P 〈 0.05）and healthy volunteers group（0.0%, P 〈 0.05）. The positive expressions of SBEM rnRNA （28.0%）and PIP mRNA（ 18.0% ）in the breast cancer group were significantly higher than those in the three groups（ P 〈 0.05）. The positive rate of CK19mRNA ＋ SBEMmRNA ＋ PIPmRNA was 62.0%. Conclusion： Both SBEM mRNA and PIP mRNA may act as markers for the identification of micrometastasis of breast cancer, whereas further studies will be needed to determine whether the CK19 mRNA is specific enough for that. The combination of three markers can enhance the positive detection of micrometastasis effectively.
