早期心理干预对脑卒中后抑郁/焦虑患者神经功能康复的影响

摘　　要：【目的】探讨早期心理干预对卒中后抑郁/焦虑患者神经功能康复的影响。【方法】采用抑郁自评量表(SDS)、焦虑自评量表(SAS)对200例脑卒中患者进行抑郁/焦虑状态评定,其中患有卒中后抑郁合并焦虑的63名患者分别分为常规治疗+黛力新治疗组(Ⅰ组)、常规治疗+黛力新联合心理干预治疗组(Ⅱ组)以及单一常规治疗对照组各21例。采用中国卒中量表(CSS)、Barthel指数(BI)、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评测治疗前后的疗效。【结果】急性脑卒中病人卒中后抑郁并焦虑患病率为31.50%。治疗Ⅰ组和治疗Ⅱ组HAMD、HAMA、中国卒中量表评分减少和BI评分增加与对照组比较均有显著性差异(P<0.001),治疗Ⅱ组HAMD、HAMA、中国卒中量表评分减少和BI评分增加较治疗Ⅰ组有显著差异(P<0.05)。【结论】脑卒中后的抑郁/焦虑病人单用黛力新治疗、黛力新联合心理干预治疗均能提高患者神经功能康复程度和生活能力恢复,而且黛力新联合心理干预治疗疗效更满意。[Objective]To study the effect of early psychotherapy for treating poststroke depression with anxiety. [Methods] In this study, 200 patients with acute brain stroke were assessed by self-rating depression scale（SDS） and self-rating anxiety scale（SAS）. Of these 200 patients, 63 patients who met the criteria of depression and anxiety after acute brain stroke/of ICD-10 were divided into Deanxit treatment group （group Ⅰ ）, Deanxit combined with psychotherapy group （group Ⅱ） and control group （group Ⅲ ）. The rating scales such as CSS, HAND, HAM, BI and so on were used to evaluate the symptoms improvement and side effects. [Results]The prevalence rate of post-stroke depressive accompanied with anxiety in acute stroke was 31.50 %. According to the decline of scores of CSS, HAMD and HAMA, and the increase of BI score of the patients, significant improvement has been showed in all patients. There were significant differences between group Ⅰ or group Ⅱ and the control group（ P 〈0.01）, and between group Ⅰand group Ⅱ （ P 〈0.05）. [Conclusion] Both single Deanxit and Deanxit combined with psychotherapy can improve neurological function and viability rehabilitation in patients with post-stroke depression with anxiety, and Deanxit combined with psychotherapy is more effective.
