超声造影测量HVTT、HTT与肝硬化Child-pugh分级的相关性研究

摘　　要：【目的】探讨低机械指数实时灰阶超声造影测定肝静脉通过时间(HVTT)、肝渡越时间(HTT)与肝硬化Child-pugh分级的相关性。【方法】研究对象分两组,研究组60例,均为慢性乙型肝炎后肝硬化患者,Child-pugh分级A级、B级、C级各20例;对照组15例,为无任何肝脏病史或肝病临床表现的健康人。所有入选对象均经左肘静脉团注SonoVue 1.6m1后,记录HVTT、HTT。【结果】HVTT、HTT的变化随Child-pugh分级的严重性加重而明显提前。HTT与Child-pugh分级进行相关性分析比HVTT意义更大。【结论】实时灰阶超声造影测定HVTT、HTT能反映肝硬化肝内血流动力学的改变,两者均对Child-pugh分级超声定性诊断具有一定的临床应用价值,其中以HTT价值更大。[Objective] To investigate the real-time gray scale contrast-enhanced ultrasonography（CEUS） for measuring HVTT and HTT and their relationship with the grade of the liver function in hepatocirrhosis. [Methods] Sixly patients with hepalocirrhosis after HBV were divided into 3 groups on the basis of liver func- tion by Child-pugh method, 20 cases in grade A, B and C group respectively. Fifteen healthy people without history or manifestation of liver disease were as control group. All of the cases were injected SonoVue 1.6ml through left elbow vein by intravenous bolus injection. HVTT and HTT were recorded. [Results]HVTT and HTT were shortened significantly with the aggravation of liver function by Child-pugh grade method. [Conclusion]Measuring HVTT and HTT by CEUS can reveal hemodynamie changes in hepatoeirrhosis. Both of them can be applied in qualitative evaluate, on of Child-pugh grade, and HTT is more valuable than HVTT.
