摘　　要：目的对改良选择性脊神经后跟部分切断术(改良SPR术)治疗痉挛型脑性瘫痪下肢痉挛的手术方法、临床疗效等相关因素综合分析,探讨痉挛型脑性瘫痪外科治疗的方法与途径。方法回顾分析我院2006年6月～2007年12月间126例采用改良SPR术治疗的脑性瘫痪下肢痉挛患者住院病历及随访资料,并做出系统评价。结果手术患者随访3～18个月,平均11.2个月。患者术后痉挛状态不同程度缓解,术后3个月生活质量改善率83.3%,随访12个月生活质量提高率91.3%,术后12个月运动功能改善率96%。结论改良SPR术疗效确切、创伤小、并发症少,是治疗痉挛型脑性瘫痪下肢痉挛行之有效的手术方式。术后加强康复功能训练,可明显改善患者症状,减轻患者个人、家庭、社会的负担,提高患者及其家庭的生活质量。Objective To aggregately analyze the oPerating method, clinical curative effect, and other related factors of improved selective posterior rhizotomy for relief of spasticity of lower lambs m cerehrral palsy patients, and in the meantime study the methods of surgical treatment for cerebral palsy. Methods Cases and fonow-up data of 126 patients in our hospital, from Jun, 2006 to Dec, 2007, who have been treated with improved SPR, were retrospectively analyzed and systematically evaluated. Results The patients had been followed up for about 3 to 18 months, and on average 11.2 months. With the treatment, the spasticity of the patients were relieved in different degrees; the third month after the operation, the improvement rate of the living standard was 83.3%; the improvement rate of the living standard after following up for 12 months was 91.3%; the twelfth month after operation, the improvement rate of motor function was 96%.Conclusion Improved SPR is of certain curative effect, small wounds and few complications, and effective for curing spasticity of lower limbs in cerebral palsy patients. Strengthening the rehabilitative management after the treatment can obviously improve the symptom of the patients, ease the burden of patients, their families and thereby the whole society, and improve the living standard of the patients and their families.
