保留肛门功能的晚期卵巢癌肿瘤细胞减灭术临床分析

摘　　要：目的:探讨运用双吻合器在晚期上皮性卵巢癌行肿瘤细胞减灭术时直肠种植瘤行直肠肿瘤切除的临床应用。方法:回顾对比性分析2001年～2005年应用双吻合器实施保肛功能的肿瘤细胞减灭术的56例和实施Miles手术的52例晚期上皮性卵巢癌患者临床资料。结果:56例患者实施Dixon手术,术后吻合瘘1例,吻合口狭窄2例,伤口感染4例,均经保守治疗治愈。19例术后有不同程度的排便习惯改变,大便频率2,3次/d,其3年生存率为48.9?,无吻合口复发。52例患者实施Miles手术3年生存率为48.2?。结论:应用吻合器对晚期上皮性卵巢癌行保肛功能的肿瘤细胞减灭术,提高生存质量未增加复发率,安全可靠,并发症少,术后生活质量高等优点,值得临床推广。Objective： To investigate the clinic significance of anus--preserving cytoreductive surgery including rectal resection for advanced epithelial ovarian carcinoma metastasizes to the rectal by using stapling device. Methods： Clinical data of anus--pre- serving eytoreductive surgery, which were performed on 56 patients with advanced epithelial ovarian carcinoma and Miles surgery which were performed on 52 patients with advanced epithelial ovarian carcinoma in 2001--2005, were retrospectively analyzed. Result.. Dixon＇s resection was performed on 56 cases, Postoperative anastomosis fistula and stenosis occurred in 1 case, 2 cases re- spectively. Wound infection occurred in 4 cases. All these complications mentioned above were cured by conservative treatment. Different degrees of alteration of defecation habit occurred in 19 cases after operation , and the defecating frequency was 2,3/d. The three years survival rates were 48.9%. Miles resection was performed on 52 cases, the three years＇ survival rates were 48. 2%. Conclusion： Anus--preserving cytoreductive surgery might be performed in treatment of advanced epithelial carcinoma by ap- plying stapling device with advantages including survival quality and doesnt increase the rate of recurrence , salty and reliability , few complication and high quality of
