门静脉插管供胃癌术后肠外营养及化疗

摘　　要：为减少中、晚期胃癌术后并发症，提高生存率，对9例患者术中行门静脉插管，供术后肠外营养及化疗用。插管后当时即自导管内缓慢推注丝裂霉素10mg；5％葡萄糖盐水500ml加5－FU1000mg滴注，维持24小时；术后第一日自插管内给肠外营养，一般支持5天，待肠功能恢复正常改口服。插管技术较复杂，但自门脉导管给养符合生理要求，且可利用导管行抗癌药物化疗，尤其对预防肝转移有一定作用。本组患者术后均恢复良好。A catheter was inserted into the portal vein during resection of the tumor in 9 patients (males 6, females 3) with metaphase or advanced gastric cancer, 10 mg of mitomycin was introduced immediately slowly through the tube. And 5% glucose saline solution with 1 000 mg. 5-FU were administered slowly for 24 hours. Intravenous nutrition started on the first day of operation for 5 days.Although this technic is rather tedious to perform, the author thinks that it is physiological. In addition, it serves also well for chemotherapy. In particular, it is promissive for the prevention of hepatic metastasis. In auther's series, all patients had uneventful recovery. Thus, the author highly recommends the application of cannulation of portal vein in suitable cases.
