前牙残根牵引种植义齿美学修复

摘　　要：目的对不能保留的前牙残根进行正畸牵引,当残根平龈时将其拔除,同期植入种植体,延期修复以达到理想的美学修复的效果。方法选择4例上前牙根折患者,断端位于牙槽嵴下4～6mm,使用正畸牵引装置对残根进行牵引;定期X线片检查根端新骨形成情况,当残根平龈后保持4～6周;拔除残根同期植入Branemark、ITI种植体共5枚,3～6月后完成贵金属烤瓷冠修复;定期复查X线片、牙龈、牙周状况。结果4例患者5个残根牵引距离3～5mm,5枚种植体随访12～24月,种植义齿稳固、美观,疗效满意。结论对前牙残根进行正畸牵引,牵引过程中可产生牙槽骨及附着龈的伴行升高,为种植义齿修复提供了充足的软、硬组织条件,在此基础上完成种植义齿具有良好的功能和美学效果。Objective To use orthodontic eruption of anterior residual fractured roots that cannot be retained before implant placement. By removing the residual root, the implant denture is placed. This postponed restoration achieves better esthetic effects. Methods For 4 cases with residual fi＇actured roots and ends 4 - 6 mm under alverolar ridge, orthodontic eruption of roots was porformed. When the roots were dragged to reach the gingiva top level, they were maintained for 4 - 6 weeks, with regular X-ray checks to verify the formation of new bone. The residual roots were removed and immediate implants were inserted with 5 Branemark and ITI implants, followed by restoration with gold-ceramic crowns in 3 ~ 6 months; and periodical rechecks were made for the gingiva and periodont. Results 5 residual roots of the 4 cases were drawn up for 3 - 5 mm, and all patients were satisfactory with the implant dentures for stable functioning and good appearance during follow-up periods from 12 to 24 months. Conclusions Orthodontic eruption of residual roots can help develop the gingiva and bone in a coronal direction before implant restoration, and augment both soft and hard tissue profiles of potential implant sites. Good function and esthetic results can be ensured for the implant denture made on this basis.
