眼科手术与全身病—1486例中老年眼科手术临床分析

摘　　要：为了解眼科手术与全身病的相互影响，统计了１４８６例５０岁以上的白内障、青光眼及视网膜脱离等常见手术的并发症及手术期间发生的全身病。结果：患有全身病者白内障手术的并发症较多。眼科手术期间全身病的发生率为５．５％，最常见者为高血压及心脏病，且多与原有的全身病有关。患有各种不同全身病者虽多达全部病例的６５．５％，但术后死亡明显与眼科手术有关者仅有１例（０．６７‰）。结论：许多有全身病的中老年患者经过必要的术前准备及良好的术中术后监护，都可以安全完成眼科手术；在监护中应特别重视高血压、心脏病。bjective:To　study　the　influence　of　ophthalmic　surgery　and　systemic　diseases　on　each　other.Methods:Clinical　analysis　of　the　surgical　complications　and　systemic　diseases　occurring　during　the　course　of　operation　of　1486　cases　of　cataract,glaucoma　and　retinal　detachment,more　than　50　years　of　age.Results:There　were　relatively　more　frequent　complications　of　cataract　surgery　in　patients　with　preexisting　systemic　disease.The　morbidity　of　systemic　disease　during　the　course　of　operation　was　55%,the　hypertension,heart　disease　had　the　highest　occurence,and　most　of　them　were　related　to　the　preexisting　systemic　diseases.Although　there　were　more　than　655%　cases　with　various　systemic　diseases,the　mortality　obviously　associated　with　ophthalmic　surgery　was　067‰,e.g.only　one　in　1486　cases.Conclusion:Most　senile　and　presenile　patients　with　systemic　diseases　could　safely　undergo　ophthalmic　surgery　after　necessary　preparation　and　under　sufficient　care　during　the　course　of　operation,with　particular　attention　to　hypertension　heart　disease　and　preexisted　systemic　diseases
