低浓度丝裂霉素C联合几丁糖对兔眼小梁切除术的抗瘢痕作用

摘　　要：目的:研究不同低浓度丝裂霉素C(MMC)联合几丁糖在小梁切除术后抗瘢痕作用及安全性。方法:30只健康新西兰大白兔均分为三组(A、B、C),随机选一眼为实验眼,另侧对照,均行小梁切除术,实验眼结膜瓣、巩膜瓣下分别应用不同浓度MMC(0.01、0.025、0.05mg/mL)及几丁糖,对照眼单用几丁糖。术后观察滤过泡形态,监测眼压及组织中成纤维细胞的变化。结果:术后B、C组中实验眼结膜下成纤维细胞数明显低于对照眼(P<0.05),C组巩膜瓣下成纤维细胞数明显低于对照眼(P<0.01);三组实验眼眼压均显著低于对照眼(P<0.05),实验眼功能滤过泡形成率与对照眼无差别(P>0.05),均无严重并发症。结论:低浓度MMC联合几丁糖能安全有效地抑制小梁切除术后纤维增殖,降低眼压。Objective To investigate the anti-cieatricial effects and safety of different low-dose mitomycin C combined with ehitosan after trabeculeetomy. Methods Thirty New-Zealand rabbits were randomly divided into three groups （ group A, B and C）. One eye of each rabbit was randomly selected as experiment eye, and the other eye was served as control, all were performed trabeculectomy. Different doses of MMC（ 0.01,0.025, 0. 05mg/mL） and chitosan were administered in eonjunetival flap and seleral flap in experiment eyes respectively, while the control eyes only used chitosan. The morphology of filtering bleb was observed, IOP and the changes of filbroblast were monitored after operation. Results The number of filbroblast in experiment eyes under conjunetival flap was significantly less than that in control eyes in group B and C（ P 〈 0.05 ）, but the number of filbroblast in experiment eyes under scleral flap was significantly less than that in control eyes in group C （ P 〈 0.01 ）. IOP was significantly lower in experiment eyes than that in control eyes in all groups （P 〈 0.05 ）. The rate of functioning bleb was no significant difference between experiment eyes and control eyes （ P 〉 0.05 ） in all groups without any severe complication. Conclusions Low-dose mitomycin C combined with chitosan can effectively and safely inhibit the proliferation of filbroblast, decrease lOP after trabeeulectomy.
