649例蒙古族患者乳腺癌ER、PR、C-erbB_2、P^(53)、PCNA的表达及预后分析

摘　　要：目的:探讨雌孕激素受体(ER、PR)、原癌基因(C-erbB_2)、抑癌基因(P^(53))、增殖细胞核抗原(PCNA)与蒙古族患者乳腺癌组织学分级、TNM分期、肿瘤大小、淋巴结转移及术后生存期的关系。方法:应用免疫组化S-P法检测649例蒙古族患者乳腺癌组织中ER、PR、C-erbB_2、P^(53)、PCNA的表达。结果:ER、PR阳性表达率随乳腺癌组织学分级升高而降低,随TNM分期升高而降低。C-erbB_2、P^(53)、PCNA阳性表达率随着乳腺癌组织学分级的升高而升高,随着TNM分期升高而升高。C-erbB_2、P^(53)、PCNASⅡ值在术后生存期<5年组表达率高,而在术后生存期≥5年组表达率低,在术后生存期<10年组表达率高,而在术后生存期≥10年组表达率低。C-erbB_2、P^(53)、PCNA与淋巴结转移个数呈正相关,与ER、PR呈负相关。结论:ER、PR是用于判断预后、指导内分泌治疗的一项重要指标。C-erbB_2、P^(53)、及PCNA可为判断预后提供依据。Objective： To investigate the relations of expression of ER,PR,C-erbB2,P and PCNA with histological grade,TNM stage,tmner volume,metastasis of the tumor in the lymph node and survival time in Mongolian patients with breast carcinoma.Methods： hrmmnohistoehemical S-P method was performed to examine ER,PR,C-erbB2,P53 and PCNA in 649 cases of breast carcinoma.Results：Positive rates of ER and PR decreased with increasing grade of breast carcinoma ,with increasing TNM stage.Positive rates of C-erbB2,P53 and PCNA inereased with increasing grade of breast cancer,with increasing TNM stage. The expression rates of C-erbB2,P53 and PCNA S Ⅱwere high in patients living for less them 5 years ＇after surgical operation,and were low in those living for t≥ 5 years after operation,were high in patients living for less than 10 years after surgical operation,and were low in those living for ≥ 10 years after operation .C-erbB2,P53 and PCNA S Ⅱ were positively correlated and ER,PR were negatively correlated with metastasis of the tumor in the lymph node .Conclusion： ER and PR are important indexes for judging prognosis and directing endocrine treatment.C-erbB2,p53 and PCNA are also faetors for judging prognosis.
