摘  要:目的:探讨多层螺旋CT(MSCT)及多种后处理技术在颈动脉体瘤(CBT)术前评价中的价值。方法:回顾性分析13例CBT的影像学资料,分析肿瘤外形、累及范围、肿瘤与毗邻血管的关系,根据临床Shamblin标准进行分型,并与术后结果对比分析。结果:13例均位于颈总动脉分叉处,平扫为密度均匀的软组织肿块,边界清楚,增强扫描呈均匀或不均匀明显强化,3例病灶内可见肿瘤滋养血管,7例早期呈斑点样强化,颈内外动脉分离,角度增大,部分病例颈动脉受压变细。根据Shamblin分型标准进行术前分型,其中Ⅰ型7例,Ⅱ型5例,Ⅲ型1例,和术后结果完全吻合,准确度为100%。结论:多层螺旋CT及后处理技术能明确CBT的定性及定位诊断,结合多种后处理技术能清晰直观地显示肿瘤与邻近血管的关系,可以进行术前临床分型,对颈动脉体瘤的手术方式有一定的指导作用。Objective: To discuss the value of MSCT findings in the diagnosis of carotid bodytumor. Methods: 13 cases with carotid body tumor by 16 MSCF enhanced CT scan were analyzed respectively. The shape, in and out vessels of angioma, relation between the angioma and encephalic structure were observed. The MSCT findings were analyzed retrospectively in comparison with operative findings of carotid body tumor. Results: Thirteen cases were all located at the level of carotid artery bifurcation. All lesions demonstrated wellmarginated masses of homogeneous softtissue. All cases were manifested as early obvious homogeneous or inhomogeneous enhancement, in which supplying and drainage vessels were displayed in 3 eases. The angle of the common carotid bifurcation was enlarged. 7,5, 1 of 13 carotid body tumors were classified as shamblin Ⅰ , shamblin Ⅱ and shamblin Ⅲtype, respechvely. The diagnosis of MSCT was confirmed by post-operation pathological sections. The accuracy rate was 100%. Conclusion: MSCT and post-processing technique have advantage in showing the shape, location, relation between the angioma and encephalic structure, MSCT can not only support the diagnosis, but also provide clinical classification for surgical plan.
