摘　　要：目的:观察家族性腺瘤样息肉病(APC)蛋白在正常胃黏膜、胃腺瘤和胃癌组织中的表达,探讨其在胃癌发生、发展中的作用。方法:取得经胃镜活检病理检查证实的78例患者的不同胃组织标本,包括24例正常胃黏膜、24例胃腺瘤性息肉(胃腺瘤)和30例胃腺癌组织,以免疫组化ABC法检测标本中APC蛋白的表达。结果:正常对照组胃黏膜、胃腺瘤、胃癌组织APC阳性表达率呈递减趋势,经两两比较,差异均有显著性统计学意义(P<0.05)。高级别上皮内瘤变APC蛋白阳性表达率虽然低于低级别上皮内瘤变,但差异无统计学意义(P=0.697)。结论:在胃腺瘤组织中APC基因表达已出现异常,该基因的失活在胃癌的发生、发展中及胃腺瘤的发生中起重要作用。Objective ： To investigate the role of adenomatous polyposis coli （APC） protein in the carci- nogenesis and progression of gastric cancer by observing its expression in normal gastric mucosa, gastric adenoma and gastric adenocarcinoma tissues. Methods： Seventy-eight gastric epithelial biopsy speci- mens were obtained, 24 from normal gastric mucosa, 24 from gastric adenomatous polyp （14 high-grade and 10 low-grade intraepithelial neoplasia） and 30 from gastric adenocarcinoma. The expression of APC was detected by immunohistochemical avidin-biotin complex （ABC） method. Results： The positive expression of APC was decreased successively from the normal control to the gastric adenomatous polyp and the gastric adenocarcinoma group, with statistically significant differences （ P 〈 0.05 ）. The high-grade intraepithelial neoplasia showed a lower positive expression of APC than the low-grade, but the difference was not statistically significant （ P = 0. 697 ）. Conclusion ： The expression of the APC gene has become abnormal in gastric adenoma, and its inactivation might play an important role in carcinogenesis and progression of gastric cancer.
