摘  要:目的:探讨非小细胞肺癌神经内分泌分化特征与细胞凋亡及凋亡相关基因Apaf-1和Caspase-3表达之间的关系。方法:对100例非小细胞肺癌手术标本应用TUNEL法检测细胞凋亡,免疫组织化学SP法检测癌组织中神经内分泌标志物神经特异性烯醇化酶、嗜铬素A、突触素及凋亡相关基因Apaf-1、Caspase-3蛋白的表达。结果:30例非小细胞肺癌具有神经内分泌分化特征,其平均凋亡指数及Apaf-1、Caspase-3蛋白表达均显著高于不伴神经内分泌分化者(P<0.05或P<0.01);Apaf-1和Caspase-3表达之间呈正相关(r=0.2877,P<0.01),两者与细胞凋亡之间亦呈正相关关系(r分别为0.3859和0.2316,均P<0.05)。腺癌和腺鳞癌组织的Apaf-1蛋白表达阳性率较高(P<0.01),Caspase-3蛋白表达阳性率在老年患者、中高分化癌和伴淋巴结转移者较高(P<0.05或P<0.01)。结论:伴神经内分泌分化的非小细胞肺癌具有较高凋亡活性,是其对放、化疗敏感的生物学基础,Apaf-1和Caspase-3高表达发挥了重要作用。Objective: To observe the relationship between the apoptosis and the neuroendrocrine differentiation in nonsmall cell lung carcinoma (NSCLC), as well as the expression of the apoptosis related genes Apaf-1 and caspase-3. Methods: One hundred cases of non-small cell lung carcinoma were examined using immunohistochemical (IHC) staining for neuroendocrine markers (chromogranin A, synaptophysin and neuron specific enolase), Apaf-1 and caspase-3 protein. Apoptosis of non-small cell lung carcinoma cells was detected by TUNEL technique. Results: Thirty cases were considered as NSCLC with neuroendocrine differentiation by the extensive and intensive staining of neuroendocrine markers, with higher apoptosis index and expression of Apaf-1 and caspase-3. Positive correlation was observed between the expression of Apaf-1 and caspase-3, as well as between either of them and apoptosis index. Apaf-1 was stained more strongly in adenoearcinoma and adenosquamous carcinoma. In contrast, caspase-3 expressed more intensively in the elders, mediate to well differentiation tumors and those with lymph node metastasis. Conclusion: Non-small cell lung carcinoma with neuroendoerine differentiation showed up-regulated apoptotic potential, which may contribute to its biological behavior such as the response to chemotherapy or radiotherapy. NSCLC expressed increased levels of Apaf-1 in mitochondria mediated apoptosome pathway and down- stream effecter caspase-3.
