进展型脑梗死的临床治疗分析

摘　　要：目的:探讨低分子肝素钙和降纤酶联合治疗进展型脑梗死的疗效。方法:将我院收治的60例进展型脑梗死患者随机分为治疗组及对照组,对照组应用低分子肝素钙,观察组在对照组基础上加用降纤酶。结果:治疗组基本治愈和显著进步的例数明显多于对照组,差异有统计学意义(P<0.05);两组治疗前后神经功能评分和日常生活能力评分均有显著性改善,差异有统计学意义(P<0.05),治疗组改善更加明显,优于对照组,差异有统计学意义(P<0.05);治疗组发生1例再梗死,占总例数的3.33%,对照组5例再次梗死,占总例数的16.67%,差异有统计学意义(P<0.05)。两组均无严重出血性并发症。结论:应用低分子肝素钙和降纤酶联合治疗进展型脑梗死早期效果显著,能改善神经功能缺损,提高日常生活能力,明显降低致残率,同时其副反应较小,值得临床推广。Objective： To discuss the clinical effect of low-molecular-weight heparin combined with defibrase in the treatment of progressive cerebral infarction. Methods： 60 cases of progressive cerebral infarction patients were randomly divided into treatment group and control group.Control group used low-molecular-weight heparin, treatment group used defibrase additionally. Results： Cases of basic cure and significant progress in treatment group were significantly more than control group, there was a significant difference between two groups（P〈0.05）; neurological score and the score of daily living signifi cantly improved before and after treatment in both two groups, there was a significant difference （P〈0.05）; the score in treat ment group improved more obvious than control group, there was a significant difference（P〈0.05）; re-infarction occurred in 1 case in treatment group（3.33 percent）, 5 case in control group（16.67 percent）,there was a significant difference（P〈0.05）. No severe bleeding complications occured. Conclusion： Combination of low-molecular-weight heparin with defibrase had obvious effect in the treatment of early stage of progressive cerebral infarction, can improve the neurological impairment, increase the activity of daily living, significantly reduce disability,at the same time,its side effect is less, worthy to be popularized.
