摘  要:目的探讨经外侧裂-岛叶入路显微手术治疗基底节脑出血的手术要点及临床疗效。方法选取我科2004年10月—2008年1月收治33例基底区脑出血患者,行经外侧裂-岛叶入路显微手术治疗。结果本组无一例出现术后出血,死亡2例,病死率6%。经随访3~12个月,按日常生活能力分级法(ADL)测评:Ⅰ级4例,占12%,Ⅱ级10例,占30%,Ⅲ级13例,占40%,Ⅳ级3例,占9%,Ⅴ级1例,占3%。结论经外侧裂-岛叶入路显微手术治疗基底节区高血压性脑出血是一种损伤轻,疗效优,预后好的微创治疗方法。Objective To study the operative techniques and therapeutic effects of microsurgical treatment with lateral fissure approach to treat hypertensive hematomas in basal ganglia region. Methods 33 cases with hypertensive hemorrhage in basal ganglia region were subjected to microsurgical treatment. The lateral fissure was opened by anatomizing sylvian cistern, after incising the cortex of insular, hemotoma was removed. Results Postoperative bleeding were not occurred in all cases,2 patients died, fatality was 6%. Activities of daily living(ADL)evaluation at 3 - 12 months follow - up showed that 4 patients were ADL1 (12 %), 10 patients were ADL2 (30% ), 13 patients were ADL3 (40 % ), 3 patients were ADL4(9%) and the other 1 case was ADLS(3%). Conclusion Microsurgical treatment with lateral fissure approach for hypertensive intracerebral hemorrhage in basal ganglia is a kind of minimally invasive method with less damage, better effects and more favorable prognosis.
