摘 要:目的探讨脱落细胞增殖细胞核抗原PCNAmRNA的检测在肺癌临床诊断中的价值。方法对48例肺癌、40例良性胸腔积液疾病患者,采用RT-PCR法检测胸腔积液脱落细胞PCNAmRNA,并与胸腔积液细胞学检查结果比较。结果PCNAmRNA检测对肺癌诊断的特异性为73.40%,低于胸腔积液脱落细胞学检测的91.34%,P<0.05,敏感性为100.00%,高于胸腔积液脱落细胞学检测的83.33%,P<0.05。肺癌及良性胸腔积液脱落细胞PCNAmRNA阳性强度分别为0.7236±0.0104、0.3352±0.0108,两者比较有显著性差异,P<0.05。胸腔积液脱落细胞PCNAmRNA阳性强度随肺癌分级增加而增强,P<0.05,与肺癌组织学类型及分化程度无关。结论胸腔积液脱落细胞PCNAmRNA的检测,在肺癌早期诊断、常规筛查以及术后复发监测中具有潜在的应用价值。Objective To evaluate the significance of detection of proliferation cell nuclear antigen (PCNA) mRNA in exfoliated cells in pleural fluid in the diagnosis of lung cancer. Methods The exfoliated cells in pleural fluid were collected from 48 patients with lung carcinoma,40 patients with benign pleural fluid. PCNA mRNA was detected by RT- PCR. Routine cytologic analysis was carried out simultaneously. Results The specificity of PCNA mRNA detection in the diagnosis of lung cancer was 73.40% ,which was significantly lower than that of cytologic analysis (91.34% ,P 〈 0.05 ) ,whereas the sensitivity of PCNA mRNA detection was significantly higher than that of routine cytologic analysis ( 100.00% and 83.33% ,P 〈 0.05 ). The intensity of PCNA mRNA in pleural fluid exfoliated cells from patients with lung carcinoma and patients with benign pleural fluid were 0. 7236 ± 0. 0104,0. 3352 ± 0. 0108 respectively. The intensity of PCNA mRNA in higher grade lung cancer was greater compared with that in lower grade cancer ( P 〈 0.05 ). Pathological differentiation and pathological types were not correlated with the intensity of PCNA mRNA. Conclusion PCNA mRNA detection by RT - PCR method has potential value in the early diagnosis, routine screening and postoperative monitoring of lung carcinoma.
