摘  要:目的:检测高分子量细胞角蛋白(34βE12)和上皮钙粘蛋白(E-cadherin)在甲状腺乳头状癌(PTC)组织中的表达,筛选甲状腺乳头状癌的辅助诊断指标及预测淋巴结转移的可能性。方法:采用免疫组化S-P法,DAB显色检测结节性甲状腺肿伴乳头状增生15例,甲状腺乳头状癌32例(无淋巴结转移22例,有淋巴结转移10例)中34βE12、E-cadherin的表达。结果:(1)34βE12表达以甲状腺乳头状癌有淋巴结转移最强(100%),无淋巴结转移较低(81.8%),甲状腺乳头状增生更低(26.7%),3组有显著差异(P<0.01)。(2)E-Cadherin表达以甲状腺乳头状增生最强(100%),甲状腺乳头状癌无淋巴结转移较低(36.4%),有淋巴结转移更低(10%),3组间有显著差异(P<0.01)。(3)根据两种蛋白表达阳性率行统计学分析,结果显示两种蛋白表达强度相互之间无相关性。结论:(1)甲状腺乳头状癌组织中34βE12表达率增高,而E-cadherin表达率低。(2)E-cadherin、34βE12对于甲状腺乳头状癌的诊断具有一定的指导作用。(3)E-cadherin有望作为预测淋巴结转移的参考指标。Objective:To examine the expression of 34βE12 and E-eadherin in the thyroid papillary carcinoma so as to screen the auxiliary diagnostic index and to predict the possibility of the transferring into lymph node. Methods: Immunohistochemical SP method was used to examine the expression of CK19,34βE12 and Ecadherin in 15 goiter with papillary hyperplasia and 32 thyroid papillary carcinoma in which 22 had lymph node transferring and 10 had not. Results: 1. The expression of 34βE12 in the thyroid papillary carcinoma with lymph node transferring is the highest(100%), low in the thyroid papillary carcinoma without lymph node transferring (81.8%) and the lowest in the thyroid papillary hyperplasia(26.7% ). The difference was significant(P〈0. 01 ). 2. The expression of E-Cadherin is lhe highesl in the thyroid papillary hyperplasia(100%), low in the thy roid papillary carcinoma without lymph node transferring (36. 4%) and the lowest in the thyroid papillary carcinoma with lymph node transferring(10 % ). The difference is significant (P〈0.01)3. According to the statistics, there is no significant difference between the expression of the two kinds of proteins. Conclusion: 1. In the tissues of papillary carcinoma of thyroids, the expression ratio of 34βE12 increased while that of E-cadherin decreased. 2. To the diagnosis of papillary carcinoma of thy roids, E cadherin and 34βE12 have instructive function. 3. E cadherin can be the index for prediction of lymphatic metastasis.
