摘  要:目的探讨幽门螺杆菌(Hp)及血清胃泌素(GAS)与十二指肠溃疡(DU)的关系。方法选择胃镜下诊断的DU患者用放射免疫法测定GAS浓度;同时用快速尿素酶试验和Giemsa染色检查是否存在Hp感染。Hp阳性组行抗Hp治疗,1个月后重复上述检查。结果Hp阳性组GAS浓度为(158.51±65.48)ng/L,显著高于Hp阴性组[(81.58±18.36)ng/L,P〈0.05]。经过1个月治疗后,Hp转阴患者血清GAS浓度为(76.82±16.09)ng/L,显著低于未转阴患者[(128.06±45.17)ng/L,P〈0.05]。结论Hp感染是DU的主要病因。Hp引起GAS增高进而造成胃酸分泌增多可能是Hp感染致病机制之一。Objective To observe the fasting serum gastrin (GAS) level in patient with duodenal ulcer which includes helicobacter pylori(Hp) positive group and Hp negative group. Methods GAS was detected with radioimmunoassay(RIA), and Hp infection was detected with rapid urease test(RUT) and Giemsa stainning in 54 patients. Repeated test was done in Hp still positive cases after one-month antibiotic therapy. Results The serum GAS level in Hp positive patients (158.51± 65.48pg/ml)was higher than that in the negative (81.58 ±18.36 )pg/ml patients (P 〈 0.05). After one-month antibiotic therapy, the GAS level of Hp positive patients (128.06 ±45.17 ) pg/ml was higher than patients whose Hp turned negative (76.82 ± 16.09 ) pg/ml ( P 〈 0.05 ). Conclusion Hp is the main causative factor in most DU eases. High gastrin caused by Hp infection producing more gastric acid may be one of the pathogeneses of Hp infection.
