摘  要:目的探讨巴曲抗栓酶对急性缺血性脑卒中病人血清补体末端复合物(TCC)和血浆纤维蛋白原(FIB)含量变化的影响。方法92例病人随机分为对照组和治疗组,2组均给予综合治疗,治疗组加用巴曲抗栓酶。在确诊当时及d 6、d 10各留取血液标本测定血清TCC水平,用药前及以后每次用药前检测FIB水平,分别于治疗前、d 7、d 14进行神经功能缺损评分。结果缺血性脑卒中发作初期血浆TCC、FIB含量2组差别无显著意义(P>0.05),2组TCC含量d 6时均增高,d 10天仍高于治疗前水平。但是在治疗后d 10治疗组TCC浓度(478±s 47)ng·L^(-1)明显低于对照组(638±39)ng·L^(-1)(P<0.01);经治疗后治疗组FIB浓度明显降低(P<0.01);治疗组神经功能缺损评分明显低于对照组(P<0.01)。结论巴曲抗栓酶能明显降低急性缺血性脑卒中病人血清TCC、FIB水平,有利于减轻炎症反应,促进神经功能恢复。[ABSTRACTI AIM To investigate the effect of batroxobin on changes of serum terminal complement comp- lex (TCC) and fibrinogen (FIB) concentrations in patients with acute ischemic stroke. METHODS Ninety-two patients were randomly divided into treatment group and control group. The combination therapy was carried out in both groups. The patients in treatment group were treated with additional batroxobin. Serum TCC concentrations were measured at the very diagnostic day and d 6, d 10 afterwards for the blood sampling test.FIB were measured before drug administration and the following every time before further administration. The neurologic impairment sores (NIS) were evaluated before the treatment and at d 7, d 14 after the treatment. RESULTS There was no significant difference in TCC and FIB concentrations between both groups during the initial stage of onset (P 〉 0.05), TCC increased significantly at d 6 and was still high at d 10 in both groups afterwards. The concentration of TCC in treatment group (478± s 47) ng·L- 1 was significantly lower than that in control group (638 ±39) ng·L-1 (p 〈 0.01) at d 10 after the additional batroxobin to treatment group. The concentration of FIB in treatment group was significantly lower than that in the control group (P 〈 0.01) . The decline of the NIS at d 7 and d 14 of the treatment group was more striking than that in the control group (P 〈 0.01). CONCLOSION Batroxobin can obviously decrease the serum levels of TCC and FIB for patients with acute ischemic stroke, and thus is favourable for.subsidence of inflammatory reaction and recovery of nerve function.
