nm23-H1与大肠癌淋巴结转移的相关性研究

摘　　要：目的：探讨ｎｍ２３Ｈ１与大肠癌淋巴结转移的关系。方法：利用限制性片段长度多态性、Ｓｏｕｔｈｅｒｎｂｌｏｔ和免疫组织化学染色技术（ＬＳＡＢ法），检测８０例大肠癌及正常粘膜ＤＮＡ中ｎｍ２３Ｈ１杂合性缺失（ｎｍ２３Ｈ１ＬＯＨ）和石蜡组织切片中ｎｍ２３Ｈ１蛋白表达。结果：８０例中４５例为杂合子，其中ＬＯＨ１４例，９例有淋巴结转移，３１例无ＬＯＨ者，仅６例有淋巴结转移（Ｐ＜０．０１）；ｎｍ２３Ｈ１蛋白表达阴性２０例中１１例有淋巴结的转移，表达阳性２５例中４例有淋巴结转移（Ｐ＜０．０１）。ｎｍ２３Ｈ１ＬＯＨ和ｎｍ２３＋Ｈ１蛋白表达阴性者在ＤｕｋｅｓＣ／Ｄ期较Ａ、Ｂ期多见（Ｐ＜０．０５）。ＬＯＨ和蛋白表达与年龄、性别、肿瘤大小、部位、组织类型无关。结论：ｎｍ２３Ｈ１ＬＯＨ与大肠癌淋巴结转移呈正相关，蛋白表达与大肠癌淋巴结转移呈负相关；检测大肠癌组织ＤＮＡ中ｎｍ２３Ｈ１ＬＯＨ和组织切片中ｎｍ２３Ｈ１蛋白表达，可能成为预测大肠癌淋巴结转移的生物学指标之一。Objective:To evaluate the relationship of nm23H1 gene and lymph node metastasis in colorectal carcinoma. Methods:Results:45 out of 80 patients were identified as heterozygosity, 14 out of 45 patients showed LOH of nm23H1 gene. The lymph node metastasis were found in 9 out of 14 patients with LOH and 6 out of 31 patients without LOH(P<0.01). Positive staining of nm23H1 protein was observed in 25 patients and negative staining in 20 patients. 4 out of 25 patients with positive staining and 11 out of 20 patients with negative staining showed lymph node metastaisis(P<0.01). LOH and negative staining of nm23H1 protein were more frequently seen in Dukes C/D stage than in Dukes A, B stage(P<0.05). LOH and expression of nm23H1 protein had no significant relation with age, sex, tumor size, location and histological type. Conclusions:LOH of nm23H1 gene were positively related to lymph node metastasis and expression of nm23H1 protein were negatively related to lymph node metastasis in colorectal carcinoma. Examining LOH of nm23H1 gene of tumor DNA and expression of nm23H1 protein on tumor sections may serve as one of the biological indicator for predicting lymph node metastasis in colorectal carcinoma. 
