全麻联合硬膜外阻滞及术后镇痛对直肠癌根治术患者循环、呼吸功能的影响  

The Influence of General Anesthesia Combined with Epidural Block and Postoperative Analgesia on the Circulation and Breathing of Patients Undergone Radical Operation for Rectal Cancer

摘　　要：为观察全麻联合硬膜外阻滞及术后镇痛对直肠癌根治术患者循环、呼吸功能的影响，将120例择期行直肠癌根治术患者随机分为3组，每组40例，A组为硬膜外麻醉联合全身麻醉加术后镇痛组，B组为双硬膜外麻醉加术后镇痛组，C组为双硬膜外麻醉无术后镇痛组。分别观察3组患者手术期及术后的血压（BP）、心率（HR）、血氧饱合度（SpO2）的变化情况。结果显示，A组患者手术期及术后的BP、HR、SpO2平稳；B组患者手术期BP下降明显，HR增快，术后的BP、HR平稳；C组患者手术期BP下降明显，术后HR增快。结果表明，全麻联合硬膜外阻滞及术后镇痛是行直肠癌根治术较为理想的麻醉方法，对循环呼吸的影响小。In order to observe the influence of general anesthesia combined with epidural block and postoperative analgesia on the circulation and breathing of rectal cancer patients undergoing radical operation, the 120 patients to be subjected the scheduled radical operation were randomly divided into three groups（40 ca ses, each group） ：A group, general anesthesia combined with epidural block and postoperative epidural analgesia;B group, double epidural block and postoperative epidural analgesia; C group, alone double epidural block. Then, their blood pressure（Bp）, heart rate（ HR）, oxygen saturation of blood （SpO2） during and after operation were observed. As results, during and after operation HR, Bp, SpO2 of the patients in A group were stable;during operation the Bp of the patients in B group were significantly decreased, their HR increased,while their postoperative Bp and HR were stable; during operation the Bp of the patients in C group were decreased significantly,while their postoperative HR increased. These results indicate that general anesthesia combined with epidurai block and potoperative epidural analgesia is more ideal anesthesia method,has a little influence on circulation and breathing.
