^(18)F-FDG PET/CT在评估乳腺癌术后转移灶治疗疗效中的价值

摘　　要：目的探讨18氟-2-脱氧葡萄糖(18F-FDG)正电子计算机断层显像(PET/CT)显像在评估乳腺癌术后转移灶治疗疗效中的价值。方法选取乳腺癌手术后半年至5年,PET/CT显像诊断有转移灶且经病理证实的病例16例,经3个月至1年的治疗后,再行第2次PET/CT显像,通过2次显像的对比,观察病灶的18F-FDG的摄取变化及形态学的改变情况来评估疗效,并与临床症状的变化进行对照。结果16例病例中,PET/CT显像为疗效好的12例中,临床症状均明显好转;PET/CT显像为疗效不好的3例,临床症状有2例好转,另1例无改善;PET/CT显像病灶增多、无疗效1例,此病例7个月后死亡;PET/CT显像的临床符合率为87.5%(14/16),阳性预测值为100%(12/12),阴性预测值为50%(2/4)。结论18F-FDG PET/CT显像在评估乳腺癌术后转移灶治疗后疗效中具有非常重要的临床价值。Objective To evaluate the clinical value of 18F-FDG PET/CT imaging in the assessment of post-treatment breast cancer metastasis.Methods 16 patients with breast cancer of six months to five years after surgery were recruited in the study,which were diagnosed by PET/CT imaging as metastasis foci and confirmed by pathological examination.After treatment ranging from three months to one year,they were rediagnosed by PET/CT imaging.Both imaging results were compared to observe the lesions of 18F-FDG metabolism changes and the morphological changes;the result was then compared with the changes in clinical symptoms.Results The PET/CT imaging of 12 out of 16 cases showed satisfactory results,the clinical symptoms were also significantly improved.PET/CT imaging of 3 cases showed dissatisfactory results,and the clinical symptoms in two of them were improved,and the clinical symptoms of one patient showed no improvement.The PET/CT imaging in one patient showed no efficacy with more foci,and the patient died seven months later.The diagnosis compatibility rate of PET/CT imaging was 87.5%,and the positive predictive value was 100%,negative predictive value 50%.Conclusion PET/CT imaging had a very important clinical value in the assessment of breast cancer metastasis after treatment.
