逆行胬肉切除联合结膜瓣翻转、桥状结膜瓣移植术治疗翼状胬肉疗效观察

摘　　要：目的观察逆行胬肉切除联合结膜瓣翻转、桥状结膜瓣移植术治疗翼状胬肉的疗效,探讨降低翼状胬肉复发率的方法。方法原发性翼状胬肉60眼,随机分为A、B两组。A组行逆行胬肉切除联合结膜瓣翻转、桥状结膜瓣移植术(30眼)。B组行单纯胬肉切除术(30眼),术后随访6～12月。术后观察角膜上皮修复时间及复发情况。结果采用逆行胬肉切除联合结膜瓣翻转、桥状结膜瓣移植术者,2眼复发;采用单纯胬肉切除术者,8眼复发。两组统计学比较,P<0.05,差异有显著意义。两组术后角膜上皮修复时间对比,P>0.05,无统计学意义。结论逆行胬肉切除联合结膜瓣翻转、桥状结膜瓣移植术能有效治疗、预防翼状胬肉的复发。Objective To investigate the therapeutic effect of retrorse excision of pterygium with fold conjunetival flap and bridge--like conjunctival flap implantation for treatment of pterygium, to approach the method to reduce postoperative recurrence rate. Methods 60 eyes of pterygium were divided randomly into two groups： retrorse excision of pterygium with fold eonjunctival flap and bridge--like conjunetival flap implantation （A group, 30eyes）,and simple pterygium excision （B group, 30eyes）. All the patients were followed up for 6-12 months. After surgery, we observed the difference of the postoperative healing time of corneal epithelium and the pterygium recurrence rate. Results 2 of 30 eyes （6.67%） in A group and 8 of 30 eyes （26.67%）in B group were recurred, the difference was very significant （P 〈 0.05）. The contrast of the postoperative healing time of corneal epithelium between group A and B was no significance, P 〉 0.05. Conelusions Retrorse excision of pterygium with fold eonjunctival flap and bridge--like conjunetival flap implantation can prevent the recurrence of pterygium. It＇s a reasonable therapeutic method for the treatment and recurrence of pterygium.
