胃癌D_3淋巴结清扫术74例临床分析

摘　　要：目的探讨D3淋巴结清扫术对进展期胃癌患者的预后、并发症和生活质量的影响。方法对2000年4月至2002年10月收治的74例进展期胃癌D3淋巴结清扫术患者进行回顾性分析。结果D2组和D3组术后2年的生存率无差别,但3至5年的生存率出现差别,D2组为58.6%、46.6%和37.9%,D3组为71.6%、64.9%、56.8%,两者之间比较P<0.05。术后并发症发生率D2组和D3组分别为8.6%和13.5%,两者之间比较无明显差异P>0.05,仅在手术时间上有所差异。D2组和D3组两组患者术后6个月,组间各项特殊症状评分指标差异无显著性意义。结论进展期胃癌D3巴结清除术是安全的,能够显著地改善患者的预后且不影响生活质量。Objective To study the influence of D3 lymph node dissection on complications, prognosis and quality of life of patients with gastric carcinoma. Methods 74 patients with advanced gastric carcinoma were subjected to D3 lymph node dissection from April 2000 to October 2002 were analyzed retrospectively. Results The two-, three-, and five-year survival rates in D2 group was 58.6% , 46.6% , and 37.9% respectively, and in D3 group was 71.6% , 64.9%, and 56.8% respectively. No significant difference was found between the two groups in the 2-year survival rates but in the 3- to 5-year survival rates （P 〈 0.05）. The complication rate of the two groups was 8.6% and 13.5% respectively, and no significant difference was found （P 〉 0.05）. The operative duration was different between the two groups. Six months after the operation, no significant difference in specific symptoms was found in both groups. Conclusions D3 lymph node dissection is safe to patients with advanced gastric carcinoma, and it can improve the prognosis effectively without the influence of quality of life.
