食管贲门癌切除机械吻合776例临床分析

摘　　要：目的评价国产吻合器在食管重建中的应用价值。方法1998年10月～2007年12月应用常州产WGW型或KYG型吻合器重建食管胃吻合口776例,其中男545例,女231例,食管癌400例,胸上段癌24例,胸中段癌290例,胸下段癌91例,多发癌5例;贲门癌376例。食管胃颈部吻合3例,主动脉弓上或胸顶吻合346例,弓下吻合428例,全胃切除12例,均行空肠代胃机械吻合。结果发生吻合口瘘4例,发生率为0.52%。1998年前手工吻合术后吻合口瘘26例,发生率为3.23%,两者相比有显著差异。吻合口狭窄28例,发生率3.61%,大部分经食管扩张症状有良好缓解。结论机械吻合重建食管安全可靠,能明显降低吻合口瘘的发生率。Objective To evaluate the application value of domestic stapler in the reconstruction of the esophagus. Methods October 1998 to December 2007 of Changzhou WGW or middle-KYG reconstruction esophagus and stomach stapling anastomotic 776 cases,of which 545 cases of male and female 231 cases, 400 cases of esophageal cancer, cancer of the chest on 24 cases, of the chest middle the 290 cases of cancer,cancer of the chest under the 91 cases,more than five cases of cancer,376 cases of cardiac cancer. Esophagus and stomach and neck anastomosis three cases,thoracic aortic arch anastomosis Top 346 cases,under the arch anastomosis 428 cases, 12 cases with full stomach,jejunum and stomach in all mechanical anastomosis. Results Anastomotic leakage results in four cases,the rate was 0.52 percent. In 1998 before the hand-anastomosis anastomotic leakage of 26 cases,the rate was 3.23 percent,compared with two significant differences. Anastomotic stenosis in 28 cases,the rate of 3.61 percent,most of the symptoms of esophageal dilatation a good mitigation. Conclusion Mechanical anastomosis reconstruction of esophageal is safe and reliable and significantly reduce the incidence of anastomotic leakage rate.
