神经母细胞瘤中NSE、S-100、CGA及p53、p21、C-erb-B_2蛋白表达与分化程度关系的研究

摘　　要：回顾性研究了４９例原诊断为神经母细胞瘤（ＮＢ），神经节神经母细胞瘤（ＧＮＢ）和神经节细胞瘤（ＧＮ）的病理资料。结果表明，这３种肿瘤为分化程度不同的同源性肿瘤，统称为神经母细胞瘤。应用免疫组化方法检测ＮＳＥ、Ｓ－１００、ＣＧＡ和ｐ５３、Ｃ－ｅｒｂ－Ｂ２、ｐ２１在该肿瘤中的表达和分布，发现ＮＳＥ、Ｓ－１００和ＣＧＡ阳性率分别为８５％、７３％和５０％，在肿瘤的Ⅰ～Ⅳ级之间，分化越好者阳性率越高（Ｐ＜０．０５），可做为临床鉴别诊断的估计预后的指标；ｐ５３、Ｃ－ｅｒｂ－Ｂ２和ｐ２１在肿瘤各级中均有表达，提示在该肿瘤发生中抑癌基因突变和原癌基因激活起一定的作用，其中低分化组ｐ５３蛋白的表达明显强于高分化组（Ｐ＜０．０５）。Forty nine cases of neuroblastoma were studied and classified into 4 grades according to their differentiation degree.Neuroendocrine markers(neuro specific enolase,S 100,Chromgranin A)and the products of oncogene(p 53 ,C erb B 2,p 21 )were examined with immunohistochemical method.The results showed 85% of tumors were positive to NSE,73% to S 100 and 50% to CgA.The rate is higher inⅢ-Ⅳ grade than that inⅠ-Ⅱ( P <0.005).All the three oncogenes were examined in tumor cells from Ⅰ Ⅳ grade and the p 53 protein expression was higher in lower differentiated tumors than that in higher ones.The neuroendocrine markers (NSE,CgA,S 100) and p 53 protein were correlated with the differentiation of neuroblastoma,and also were helpful factors in evaluating the prognosis of the tumor.
