丝裂霉素C联合20%乙醇分离翼状胬肉切除术的临床观察

摘　　要：目的研究20%乙醇联合丝裂霉素C翼状胬肉切除术的疗效。方法翼状胬肉患者82例(88眼),随机分组后分别行20%乙醇分离联合丝裂霉素C翼状胬肉切除术(A组43例,47眼)和显微切除联合丝裂霉素C翼状胬肉切除术(B组39例,41眼)。结果术后A、B组角膜创面平均上皮修复时间分别为2.55d和4.90d,差异有统计学意义(P<0.01)。A组角膜创面光滑透明,泪膜形成良好,结膜平整,平均随访8个月,44眼治愈,2眼结膜增厚复发,1眼并发结膜囊肿。B组36眼治愈(87.80%),5眼复发,差异有统计学意义(P<0.05)。结论丝裂霉素C联合20%乙醇翼状胬肉切除术创伤少、并发症少、复发率低、安全性高,不破坏眼部正常生理功能,能恢复正常、健康眼表结构,是治疗翼状胬肉的新方法。Objective To evaluate the effects of 20% alcohol combined with mitomycin C for the treatment of pterygium. Methods Eighty-eight eyes of 82 patients with pterygium were randomly divided into 2 groups for the treatment of 20% alcohol combined with mitomycin C （group A,47 eyes）and mierosurgery with mitomycin C （group B,41 eyes）. Results After the operation, the average epithelial recovering periods of the corneal operating field were 2.55 days in group A and 4.90 days in group B（P 〈0.01 ）. The operating field including the cornea and bulbar conjunctiva of group A appeared smoothy and transparent after the operation. An average follow-up of 8 months was done after the operation in all patients. Forty-four eyes of group A （93.62%） were cured,while recrudescence happened in 2 eyes and concurrent conjunctival cyst in 1 eye. Thirty-six eyes were cured in group B（87.80% ） , and 5 eyes suffered relapses. The cure rate was 93.62% in group A and 87.80% in group B, with significant difference between them（P 〈0.05）. Conclusions Comparing with the traditional pterygium excision, the new method has the advantage of less complications, lower recurrence rate and higher security.
