摘  要:目的:评价肩台宽度≤0.5mm+金属颈环对后牙烤瓷熔附金属修复体崩瓷发生的影响。方法:将近5年来收集的3 697颗后牙金瓷修复体按照颈缘设计的不同分为3组:A组为标准肩台组;B组为肩台宽度≤0.5 mm;C组为肩台宽度≤0.5mm+270°或360°全金属颈环,比较各组崩瓷发生率和各组不同轴面崩瓷构成比的情况。结果:A组和C组的崩瓷发生率以及各轴面崩瓷的构成比与B组的差异均有统计学意义(P<0.05),而A组和C组的崩瓷发生率以及各轴面崩瓷的构成比的差异无统计学意义(P>0.05)。结论:肩台宽度≤0.5 mm+270°或360°全金属颈环设计的后牙金瓷修复体可减少崩瓷发生率,这对于减少牙体组织制备、降低金瓷修复失败有指导意义。Objective: This follow-up study is designed to evaluate the effect of the shoulder width ≤ 0.5 mm plus cervical metal collar on the occurrence of porcelain fracture on posterior PFM restorations. Methods : A total of 3 697 posterior PFM restorations collected from recent 5 years were divided into 3 groups according to their different cervical designs: standard shoulder (90° or 135°) in Group A, shoulder width ≤ 0.5 mm in Group B and shoulder width ≤ 0.5 mm and 270° or 360° cervical metal margin of PFM in Group C. Chi-square test was applied to the analysis of the differences in the incidence of porcelain fracture and the ratio of porcelain fracture on different axial surfaces among the three groups. Results : There were statistically significant differences between Group A and B in the incidence of porcelain fracture and the ratio of porcelain fracture on different axial surfaces (P 〈 0.05), but not between Group A and C (P 〉 0.05). Conclusion : The design of shoulder width ≤0.5 mm plus 270° or 360°cervical metal margin of PFM for posterior teeth can reduce the incidence of porcelain fracture and help to reduce the dentin volume of tooth preparation and decrease the failure possibility of PFM restorations.
