CRP与T-淋巴细胞亚群联检在胃癌分期及炎症反应中的意义

摘　　要：目的联合检测CRP及T淋巴细胞亚群,探讨其在胃癌分期和炎症反应中的意义。方法手术前及术后7天检测78例胃癌患者外周血中的T淋巴细胞亚群与CRP水平,分析与胃癌临床分期及感染发生的关系。结果Ⅰ、Ⅱ期胃癌淋巴细胞亚群术前术后改变不明显,Ⅲ、Ⅳ期患者淋巴细胞亚群术前明显下降,术后7天增高明显(P<0.05),Ⅲ、Ⅳ期感染率明显高于Ⅰ、Ⅱ期。术前各期胃癌的CRP改变不明显,但术后7天Ⅲ、Ⅳ期胃癌上升明显(P<0.01),感染率也高于Ⅰ、Ⅱ期(P<0.05)。结论T淋巴细胞亚群及CRP在一定程度上反映了胃癌的病期。病期越晚,CRP的免疫放大作用越明显,对组织免疫损伤程度越明显。Objective To discuss the significance of combined determination of CRP and T lymphocyte subsets in staging of gastric cancer and inflammatory response. Method The levels of serum C-reaction protein（CRP） and T lymphocyte subsets were examined in all 78 patients with gastric cancer before and 7 days after the oper- ation. Their relationship with the staging of gastric cancer and infection incidence was analyzed. Results T lymphocyte subsets were not significantly different in stages Ⅰ and Ⅱ gastric cancer before and after the operation. In patients in stages Ⅲ and Ⅳ, it decreased significantly before the operation,and increased significandy 7 days after the opertion （ P 〈 0.05 ）. The infection rates of stages Ⅲ and Ⅳ were significantly higher than those of the stages Ⅰ and Ⅱ. CRP level was not significanly different in all stages of gastric cancer before the operation,but at the 7th day after the operation, it increased significathy in stages Ⅲ and Ⅳ, the infection rates were also significantly higher than those in stages Ⅰ and Ⅱ （P 〈 0. 05）. Conclusions CRP level and T lymphocyte subsets were useful parameters to inflect the severity of gastric cancer. Along with the rise of stage, the immune amplification effect of CRP is more distinct, and the immunologic injury of tissues is more significant.
