综合治疗与封闭、免疫法治疗口腔扁平苔癣的临床研究

摘　　要：目的探讨治疗口腔扁平苔癣的最佳方法。方法随机将300例扁平苔癣患者分为3组:免疫组、封闭组、综合组。治疗并观察各组的总有效率及复发率。免疫组使用左旋咪唑、多抗甲素口服治疗。封闭组采用2%利多卡因加5%泼尼松龙液等量封闭病变基底和神经末稍治疗。综合组将以上两种方法加上消除口腔及牙齿局部不利因素(如使用0.5%碘伏消毒口腔,洁刮治牙减少菌斑、牙垢,调合牙尖,拨除残根)的治疗方法。结果①免疫组总有效率58%,封闭组总有效率66%,综合组总有效率91%。综合组与其它两组比较差异有统计学意义(P<0.01);②免疫组复发率33%,封闭组复发率45%,综合组复发率12%。综合组与其它两组比较差异有统计学意义(P<0.01)。结论综合治疗法对口腔扁平苔癣的疗效明显优于免疫法和封闭法。Objective To study the best methods in treating lichen planus.Methods 300 eases of oral lichen planus were randomieally distributed three groups：immunotherapy,bleek therapy and general therapy. Total efficacy rates and recurrence rates of every group were clinically observed.The OLP were treated with levamisole and polyresistin by taking orally in immunothearpy, with 2% lidoeaine and equal dose of 5 % prednisolone by blocking the base of pathological changes and nerve end in block therapy. In general thera- py, above the two methods combined with such methods as sterilizing mouth by 0.5% iedophor, decreasing tartars and germs, mediating teeth tips, removing damaged roots were used.Results （1）Total efficacy rates of oral llehen planus from immunotherapy, block therapy and general therapy groups were 58% ,66% and 91% respectively. Compared with the immunotherapy and bleek therapy, the rotes of total effieaey was signifieanfly higher in the general therapy（ P 〈 0.005）. （2）Reeurrenee rates from these three groups were 33 %, 45 % and 12% aeeordingly.Tne rates of recurrence was significantly lower in the general therapy（ P 〈 0.001）. Conclusion General therapy was more effeetive than immunotherapy and block therapy in treating oral lichen planus.
