408例缺血性脑血管病二级预防抗血小板药物应用状况调查

摘　　要：目的了解北京市部分二级医院缺血性脑血管病二级预防抗血小板药物的应用现状，为改进缺血性脑血管病二级预防工作提供依据。方法本研究为现况调查，研究对象为以自愿形式参加的北京市4家二级医院中急性脑梗死及短暂性脑缺血发作（TIA）的住院患者，调查其住院期间以及出院3个月二级预防抗血小板药物的应用现况。结果入选患者共458例，其中408例完成3个月随访。住院期间抗血小板药物应用率为93．7％，3个月随访发现，抗血小板药物的院外治疗依从性差，应用率明显下降．为69．6％（P=0．003）；男性（OR=1．708，95％CI 1.083～2．691）及TIA患者（OR=1．954，95％ CI 1．046～3．649）成为患者抗血小板药物依从性差的促进因素。结论北京市部分二级医院缺血性脑血管病二级预防抗血小板药物的应用现状不容乐观。临床医生应对缺血性脑血管病患者二级预防抗血小板药物的依从性给予关注。Objective To investigate the status of the using of antiplatelet in secondary prevention of cerebral ischemia and to provide clues for improving the second prevention. Methods The study was a crosssectional investigation. All inpatients in diagnosis of acute cerebral infarction or TIA（transient isehemia attack） from four second class hospitals in Beijing were recruited. The status of the using of antiplatelet in secondary prevention of cerebral ischemia during hospitalization and at 3-month after discharging were investigated. Results 458 patients met the recruiting criteria,and 408 cases of them got a 3-month follow-up. The using proportion of antiplatelet was 93.7% during hospitalization and 69.6% at 3-month follow-up, which showed a bad compliance. Male （odds ratio [OR]=1. 708, 95% CI 1. 0834-2.69]）and TIA patients （OR=1.954, 95% CI 1.046-3.649） could decrease the compliance of antiplatelet. Conclusion The status of the using of antiplatelet in secondary prevention on acute ischemic stroke and TIA in second class hospitals in Beijing was not optimistic. Doctors should pay attention to the compliance of antiplatelet in secondary prevention of cerebral ischemia.
