摘 要:目的比较免疫组织化学和逆转录-聚合酶链反应(RT—PCR)方法对结直肠癌淋巴结微转移病灶中CK20的检测价值。方法对40例结直肠癌患者共326例经HE染色证实无转移的淋巴结,使用RT—PCR技术检测CK20mRNA,并与免疫组织化学染色检测方法进行比较,以寻找更为敏感的淋巴结微转移检测方法。结果免疫组织化学法和RT—PCR分别发现41枚和105枚微转移淋巴结,两种方法发现的微转移阳性率分别为12.8%和32.2%,两者差异有统计学意义(P〈0.05)。结论RT—PCR法是比免疫组织化学染色法更为敏感的检测结直肠癌淋巴结微转移的方法。Objective To compare the diagnostic value between immunohistochemistry and RT-PCR in detecting micrometastases of lymph nodes of colorectal cancer. Methods 326 lymph nodes from 40 colorectal cancer patients with histologically netative node were studied. A immunohistochemistry was performed by the streptavidin peroxidase conjugataed method using a monoclonal antibody specifically against cytokeratin 20 (CK20) , and RT-PCR was used to detect the expression of CK20 mRNA in regional lymph node. Results Forty-one and 105 lymph nodes were found to have micrometastases by immunostaining with CK20 antibody and CK20 mRNA RT-PCR ,and the positive rate was 12.8% and 32.2% ,respectively (P 〈 0.05). Conclusion RT-PCR is more sensitive than immunohistochemistry for detection of micrometastases in lymph nodes of colorectal cancer with CK20 as marker.
