舟山群岛渔民2772例消化性溃疡的流行病学分析

摘　　要：目的：了解舟山群岛渔民消化性溃疡发病（PU）特点。方法：对10年间舟山渔民经胃镜检查诊断为消化性溃疡的病例进行统计分析。结果：共检出2772例消化性溃疡患者。其中，十二指肠球部溃疡（DU）1846例，胃溃疡（GU）854例，复合性溃疡（CU）72例。DU患者的平均年龄36．2岁，GU患者的平均年龄为44．8岁，CU患者的平均年龄44．3岁，与当地普通男性居民相比，DU的发病年龄要提早5～10年，GU提早3～5年，而CU基本一致。DU和GU的四季检出率均以秋冬季节为高。DU在PU中所占比例为66．5％，结论：（1）渔民的PU检出率及幽门螺旋杆菌感染率较高。（2）病变好发部位以球部溃疡最多见，GU次之，CU最少。（3）PU与渔民在海上工作的年限、季节、生存环境和不良生活习惯密切相关。Objective： To explore the characteristic of peptic ulcer（PU） in Zhoushan island. Methods： PU cases during the recent 10 years were analyzed by statistic methods. Results： Two thousand seven hundred and seventy- two PU eases were detected. There were 1 846 eases in duodenal bulb ulcer（DU）, 854 in gastric ulcer（GU）and 72 in complexity uleer（CU）. The average age of patients with DU was 36.2 years old, GU was 44.8 years old and CU was 44.3 years old, respectively. Ages of onset of DU was 5 - 10 years earlier and GU was 3 - 5 years earlier than those in other normal male residents. Both DU and GU had the highest detectable rate in autom and winter. There was 66.7 % DU in PU cases of fishers, which were obviously higher than those of other normal male residents. Conclusion： （1） The deteetalbe rates of PU and HP in fishers are higher than those in other normal male residents. （2） The order of predisposed sites are observed as DU, GU and CU. （3） The incident of PU is associated with time, season, environment and individual＇s living habits.
