受侵肠段切除与联合化疗治疗卵巢癌肠转移62例分析

摘　　要：１９８３年３月至１９９６年２月间，在施行肿瘤细胞减灭术中，为６２例因转移瘤侵犯深肌层或侵透肠腔的卵巢癌做了受侵肠段切除术。包括上皮性癌５５例，恶性生殖细胞瘤４例，性索间质瘤３例。其中Ⅱ期１例，Ⅲ期３９例，Ⅳ期２例，复发癌２０例。术式主要为小肠部分切除、结肠部分切除及乙状结肠直肠部分切除术。术后给以顺铂为主的联合化疗。４８４％残余肿瘤小于２ｃｍ，２年及５年存活率为４２９％与１６％，残余肿瘤小于２ｃｍ、化疗超过６个疗程者生存率高。结果提示：受侵肠段切除与化疗相结合能改善此类患者预后。bjective:To study the treatment of ovarian cancer patient with intestinal metastasis that involved the deeper uscle or lumen of the bowel.Design:The authors performed bowel resection on 62 cases of ovarian cancer for the purpose of cytoreduction during the period from March 1983 to February 1996.This paper included primary ovarian cancer 42 cases(stage Ⅱ one,stage Ⅲ 39 and stage Ⅳ 2 cases)and recurrent cancer 20 cases,including epithelial carcinoma 55 cases,malignant germ cell tumor 4 cases and sexcord stromal tumor 3 cases.Main types of operation was small intestinal resection in 6 cases,colon resection in 9 cases and rectosigmoid resection in 47 cases.All cases treated by Cisplatin based combined adjuvant chemotherapy.Result:Till the end of 1995,thirty five patients was followed up more than 2 years and twenty five patients more than 5 years.The data showed that the residual tumor volume was less than 2 cm in 48.8%(30/62),the 2 year and 5 year survival rates were 42.9%(15/35) and 16%(4/25) respectively.Patients receiving more than 6 cycle's chemotherapy had higher survival rate(P<0.05) than that of patients of receiving less than 6 cycle's.Conclusion:The results suggest that the combined bowel resection and intensive adjuvant chemotherapy can be safely performed on ovarian cancer patients with intestinal invasion,and improve their survival period. \ \
