BAG-1在非小细胞肺癌中的表达与含铂方案化疗敏感性的关系

摘　　要：目的：研究BAG—1在非小细胞肺癌（non—small celll ungcancer，NSCLC）中的表达与临床特征及含铂方案化疗敏感性的关系，探讨NSCLC的铂类耐药机制。方法：应用免疫组化的方法检测125例NSCLC术后石蜡标本中BAG—1蛋白的表达情况，其中75例术后给予含铂方案化疗、50例单独行手术治疗，随访后进行生存预后分析。结果：1）BAG-1在NSCLC中存在高表达（阳性率为64．8％）；2）BAG—1表达水平与组织学分级相关，分级越差其阳性率越高（P=0．024、0．042），与其它临床特征之间差异均无统计学意义；3）BAG-1阳性组具有生存优势，且在胞浆中的表达具有更低的死亡风险（P=0．039）；4）BAG-1阳性纽在接受含铂方案化疗后不能明显延长生存时间，阴性纽更能从中受益（P=0．048）。结论：BAG-1与铂类药物化疗敏感性相关，可能成为新型的临床预后指标，有助于NSCLC个体化治疗方案的选择。Objective： To detect the correlation of the expression of BAG-1 with clinical features and sensitivity to platinum-based chemotherapy of non-small cell lung cancer （NSCLC）, and to further investigate the mechanism of resistance to platinum. Methods： The expression of BAG-1 was measured by immunohistochemistry in paraffin sections of 125 NSCLC patients treated with surgery. Seventy-five of the 125 patients received postoperative platinum - based chemotherapy. Results： BAG-1 was highly expressed in NSCLC samples, with a positive rate of 64.8%. BAG-1 expression was correlated with histological grade. Samples from patients with poorer histological grade had higher BAG-1 expression. No correlation was found between BAG-1 expression and other clinical characteristics. Patients with BAG-1 expression had better survival compared with those without BAG-1 expression （P=0.039）. Platinum-based chemotherapy didn＇t not prolong survival time in patients with BAG-1 expression but benefited those without BAG-1 expression （P=0.048）. Conclusion： BAG-1 protein expression is correlated with the NSCLC patients＇ sensitivity to platinum-based chemotherapy. BAG-1 protein can be a new prognostic indicator and is helpful for developing individualized treatment for NSCLC.
