摘  要:为了解P糖蛋白(Pgp)在原发和复发胰腺癌组织中的表达及其临床意义,并分析其表达与p21ras和p53蛋白表达的关系。采用免疫组化法对48例原发胰腺导管癌(其中18例为胰腺癌淋巴结转移灶)和15例复发胰腺癌的石蜡包埋标本进行Pgp、p21ras蛋白和p53蛋白单抗的免疫组化染色。其结果为48例原发胰腺癌(其中18例淋巴结转移灶)和15例复发胰腺癌组织中呈Pgp染色阳性分别为21例(43.8%),7例(38.9%)和11例(73.3%),复发胰腺癌组织Pgp阳性率高于原发癌(P<0.05)。Pgp阴性组的原发癌患者经根治手术后平均生存期明显较阳性组长(P<0.01)。另外,原发癌Pgp阳性组与p21ras阳性组有相关性(P<0.05)而与p53阳性组则无相关性。结果提示Pgp表达与胰腺癌对化疗的耐药性有关。To investigate the expression of P glycoprotein (Pgp) in primary, metastatic and recurrent pancreatic adenocarcinomas; and to correlate P gp expression with p21 ras and p53 protein expression, histological samples from 48 primary pancreatic adenocarcinomas, 18 metastatic lymph nodes and 15 recurrent pancreatic cancers were studied immunohistochemically for the expression of Pgp, p21 ras and p53 proteins. Results showed membrane bound positivity of Pgp was observed in 21(43.8%) primary pancreatic adenocarcinomas, 7(38.9%) metastatic lymph nodes and 11(73.3%) recurrent cancers respectively. Expression of Pgp in recurrent pancreatic adenocarcinomas is higher than that in primary ones ( P <0.05). Among all the patients with primary pancreatic cancers, the mean survival time of negative group is obviously higher than that of the positive one. Besides, Pgp positive group was correlated with p21 ras positive group, while not correlated with p53 positive one. The results suggested (1)Pgp expression might be associated with clinical chemoresistence of pancreatic cancers. (2)Chemoresistence of p21 ras positive pancreatic cancers be probably mediated by Pgp expression.
