淋巴因子活化杀伤细胞与重组白细胞介素-2预防胃癌术后腹膜转移的研究

摘　　要：目的证实淋巴因子活化杀伤细胞(LAK 细胞)与重组白细胞介素-2(rIL-2)对胃癌术后腹膜种植转移的预防及治疗作用。方法在裸小鼠腹腔接种胃癌细胞形成腹膜种植转移模型,于不同阶段在裸小鼠腹腔内灌注不同剂量的 LAK 细胞及 rIL-2;并对胃癌根治术后27例患者腹腔应用 LAK 细胞加 rIL-2。结果在裸小鼠腹腔接种胃癌细胞的同时、显微镜下出现癌微灶及肉眼可见癌微灶三个阶段腹腔内注入一定量的 LAK 细胞,其预防种植转移的效果递减。接种癌细胞的数量减少,增加 LAK 细胞的数量及并用 rIL-2时预防效果最佳。临床应用 LAK 细胞加 rIL-2组患者出现腹膜种植转移者仅为11.1%,而腹腔应用5-Fu 组和对照组患者腹膜种植转移分别为35.5%及40.0%。结论胃癌根治术后腹腔灌注LAK 细胞加 rIL-2能有效预防腹腔种植转移。Objective To evaluate the effect of LAK cells plus rIL-2 in the prevention and treatment of postoperative peritoneal metastasis in nude mouse model of gastric carcinoma.Method After metastasis model of nude mice was established by the seeding of gastric cancer cells,LAK cells and rIL-2 of different dosage were intraperi- toneally instillated.The effects of LAK cells plus rIL-2 were also observed in 27 patients of gastric carcinoma undergoing surgery.Result It was showed that early combined use of LAK cells plus rIL-2 at large dosage was most effective in prevention and treatment of peritoneal metastasis in nude mouse model.In clinical study,it was demonstrated that only 11.1%(3/27)of the patients in the LAK group developed peritoneal metastasis which was significantly lower(P<0.05)than 5-FU infusion group (35.5%) and control group (40.0%).Conclu- sion The result suggested that instillation of LAK cells and rIL-2 can effectively prevent peritoneal metastasis after radical gastrectomy.
