不同光纤钬激光治疗非肌层浸润性膀胱癌的临床观察

摘　　要：目的比较端射光纤钬激光膀胱肿物切除术与侧射光纤钬激光膀胱肿物气化术治疗非浸润性膀胱癌的临床效果。方法将膀胱肿瘤患者随机分为3组,分别用不同光纤钬激光治疗,记录、测定3组患者术前、术中和术后的相关指标,并进行统计学分析。结果端射光纤钬激光组手术时间20～60min,平均30min,无膀胱穿孔,术后无需要膀胱冲洗,尿管留置时间1～3d,随访6个月～3年,尿道狭窄1例,复发5例;侧射光纤钬激光组手术时间15～55min,平均28min,无膀胱穿孔,无需要膀胱冲洗,尿管时间1～3d,随访6个月～3年,无尿道狭窄,复发4例;同时应用两种光纤钬激光组手术时间18～50min,平均22min,无膀胱穿孔,术后无需要膀胱冲洗,尿管留置时间1～3d,随访6个月～3年,无尿道狭窄,复发1例。3组平均手术时间,尿管留置时间、术后肿瘤复发,尿道狭窄发生率、膀胱穿孔例数差异均无显著性(P>0.05)。结论钬激光是治疗非肌层浸润性膀胱癌安全的方法,两种光纤同时应用效果更优越。【Objective】To compare the clinical efficacy of different fiber holmium laser resection for superficial bladder cancer.【Methods】Three groups of patients with superficial bladder cancer were underwent.Different fiber holmium laser treatment,different clinical aspects of the tumors and recurrences were considered.No significant difference between the three groups was noted regarding number,progression of grade and stage and place and time of recurrences.【Results】In the different fiber holmium laser patients,no perioperative complications occurred.Also,the catheter was removed within 1～3 days.The mean operative time was 30 min /28 min /22 min.And the case developed recurrence was 5/4/1.【Conclusion】Different fiber holmium laser is safe and effective for the treatment of bladder tumors and two fibers are more advanced.
