摘　　要：[目的]探讨抑酸剂、镇静剂、复方中药对大鼠应激性溃疡模型保护作用的机制。[方法]实验大鼠分为5组,各12只。中药组每天灌服复方中药心身1号2 ml,造模组及空白组灌等量0.85%氯化钠,共7 d;雷尼替丁组、阿普唑仑组于施加应激前0.5 h分别给予含各自成分的悬浊液2 ml。然后对给药4组水浸束缚,使之产生中医"气机失调"证和现代医学"应激性溃疡"的模型。比较5组胃黏膜的溃疡指数(UI)、病理及脑垂体中去甲肾上腺素(NE)、多巴胺(DA)、乙酰胆碱(Ach)的水平;检测造模组、中药组及空白组血清中免疫球蛋白A(IgA)、胃液中分泌型免疫球蛋白A(SIgA)的水平。[结果]造模组UI、NE及DA与空白组比较明显增高(P<0.01)、Ach明显降低。中药及雷尼替丁可明显降低UI及NE;雷尼替丁有明显降低DA的作用;阿普唑仑有升高Ach作用。造模组大鼠血清中IgA及胃液中SIgA的水平与空白组比较显著降低(P<0.01)。心身1号可显著提高造模组血清中IgA及胃液中SIgA的水平。[结论]各种药物通过不同的干预途径,保护胃黏膜、降低溃疡的形成。复方中药心身1号可明显改善应激导致的机体局部及全身免疫抑制状态。[Objective]To explore the mechanism of protective effect on stress ulceration in rats from the acid suppression, sedative and traditional Chinese medicine （TCM）. [Methods]Sixty rats were divided randomly into five groups （n = 12）, including normal group, model group, TCM group,ranitidine group and alprazolam group. The TCM group were fed with 2ml Xinshenl per day for 7 days, the normal group and model group were fed with 2ml 0. 9% brine per day for 7 days, ranitidine group and alprazolam group were given 2ml ranitidine and alprazolam before inducing stress ulceration. The stress ulceration of the model group, TCM group, ranitidine group and alprazolam group was induced by water immersion and immobilization. Ulcer index and pathology of gastric mucosa, norepinephrine （NE） and dopamine （DA） and acetylcholine （Ach） in pituitary were compared between groups. ImmunoglobinA（IgA） in blood serum and secretory immunoglobinA（SIgA） in gastric fluid were compared between normal group and model group and TCM group. [Results] Compared with normal group, UI and NE and DA of model group were increased obviously（P〈0.01）, Ach was decreased obviously. TCM and ranitidine could decrease UI and NE obviously, ranitidine could decrease DA obviously, alprazolam could increase Ach; compared with normal group , IgA in blood serum and secretory SIgA in gastric fluid of model group were decreased obviously（P〈0. 01）. Xinshenl could increase obviously IgA in blood serum and secretory SIgA in gastric fluid. [Conclusion ]Acid suppression, sedative and Xinshenl can reduce the occurrence of stress ulceration by different mechanisms. Xinshenl can decrease obviously local and systemic immunosurpression.
