摘  要:目的:研究Fas、FasL和Caspase-3在肺癌组织中的表达及其临床意义,并探讨其在肺癌细胞免疫逃逸中的作用。方法:采用免疫组化法检测52例肺癌组织,18例正常支气管黏膜上皮组织Fas、FasL和Caspase-3蛋白表达情况。结果:Fas、FasL与Caspase-3在肺癌组织中的表达率显著低于正常支气管黏膜上皮(P<0.01),在肺鳞癌与肺腺癌中无显著性差异(P>0.05),非小细胞肺癌Fas与Caspase-3的阳性表达率高于小细胞肺癌,差异有显著性(P<0.05),但FasL在非小细胞肺癌与小细胞肺癌中的阳性表达率无显著差异(P>0.05);随肿瘤病理分级增高,Fas、FasL和Caspase-3阳性表达率均逐渐减低(P<0.05),但Fas、FasL和Caspase-3的表达与淋巴结转移、TNM分期无关(P>0.05)。肺癌组织中Fas、FasL与Caspase-3的表达相互间并无关联性。结论:肺癌能通过Fas、FasL与Caspase-3的表达下调,逃避机体免疫监视以促使肿瘤发生发展。Objective:To investigate the expression and significance of Fas , FasL and Caspase -3 in lung cancer and explore the mechanism of tumor immune evasion. Methods: Immunohistochemical SP method was used to examine the expression of Fas , FasL and Caspase -3 in 52 cases of lung cancer and 18 cases of normal epithelium tissue of tunica mucosa bronchiorum. Results: The positive expression rates of Fas, FasL and Caspase - 3 were significantly lower in lung cancer than in normal epithelium tissue of tunica mucosa bronchiorum ( P 〈 0.01 ). The positive expression of Fas, FasL and Caspase -3 were different, though insignificantly, between squamous cell carcinoma and adenocarcinoma . The positive expression of Fas and Caspase - 3 in NSCLC were significantly higher than that in SCLC(P 〈0.05). The positive expression rates of FasL in NSCLC and SCLC were 59.18% (29/49) and 33.33% (1/3) respectively , there was no significant difference between them (P 〉 0.05). The positive expression of Fas, FasL and Caspase -3 in lung cancer were associated with pathological grade( P 〈 0.05 ), but none of them had any correlation with local lymph node metastases or TNM stages. There was no correlation between Fas expression and FasL expression in lung cancer, and had the same result between Fas and Caspase -3, or FasL and Caspase -3 . Conclusion: Down - regulation of Fas, FasL and Caspase - 3 may contribute to the carcinogenesis and development of lung carcinoma by evading the host immune surveillance.
