p27与MMP-9在直肠癌中的表达及临床意义

摘　　要：目的:探讨p27蛋白和基质金属蛋白酶9(MMP-9)在直肠癌中的表达特点及其临床意义。方法:采用免疫组织化学Envision二步法检测254例直肠癌组织标本中p27蛋白和基质金属蛋白酶9的表达,并分析与临床病理因素的相关性。结果:p27蛋白阳性表达率为75.2%(191/254),低表达率为67.72%(172/254),核质p27蛋白共表达85例。MMP-9阳性表达率为85.04%(216/254),高表达率为68.11%(173/254)。p27蛋白和MMP-9的表达与直肠癌的组织学类型、Dukes分期、淋巴结转移、浸润深度、外周血CEA水平、生存期密切相关(P<0.05),而与肿瘤部位、肿瘤大小、病理类型、脉管瘤栓、患者年龄性别无关(P>0.05)。结论:直肠癌中p27低表达和MMP-9高表达,可成为判断直肠癌侵袭、转移及预后的指标。Objective ：To investigate the expression of p27 protein and MMP -9 （matrix metalloproteinase- 9, MMP- 9） and correlation between them in rectal cancer. Methods： The expressions of p27 and MMP -9 in 254 rectal cancer patients were detected by immunohistochemistry, and then the correlations between them and clinical pathological parameters were analyzed. Results： The positive p27 expression rate was 75.2% （ 191/254 ） , while low expression rate was 67.72% （172/254）. Of 85 rectal cancer, p27 expression was positive both in nuclear and cytoplasm. The positive MMP -9 expression rate was 85.04% （216/254） ,while high expression rate was 68.11% （173/ 254）. p27 and MMP- 9 expressions had significant correlation with differentiation extent, Dukes staging,lymphnode metastasis and distant metastasis, infiltration depth, CEA levels and survival period （P 〈 0.05 ）, but no correlation were found with tumor location, tumor size, pathologic type, tumor embolus in vessels,patient age and gender （ P 〉 0.05 ）. Conclusion ： Down - expression of p27 and over - expression of MMP - 9 could be indicators to judge the invasion, metastasis and prognosis of rectal cancer.
