子宫内膜良恶性病变的经阴道彩超与病理的对比分析

摘　　要：目的评价经阴道彩超对子宫内膜良恶性病变的诊断价值。 方法对手术病理证实的136例子宫内膜良恶性病变的经阴道彩超声像图进行回顾性分析，并与病理结果对照。 结果经阴道彩超对于子宫黏膜下肌瘤、子宫内膜息肉、子宫内膜癌的病理诊断符合率较高，分别为87．5％、90．0％及80．0％。通过经阴道彩超对内膜回声、厚度的观察测量，有助于临床对萎缩性内膜炎、子宫内膜简单型增生过长的诊断及用药，可避免不必要的诊断性刮宫。增厚内膜的低速低阻血流的存在对子宫内膜癌的诊断有重要作用，而根据内膜与肌层之间低回声带的中断与否进行内膜癌的肌层浸润评估对手术及预后具有重要意义。 结论经阴道彩超对于子宫内膜良恶性病变的鉴别诊断具有较高的价值，对于一些具有交叉重叠性的声像图需加以临床综合分析以提高诊断准确率。Objective To evaluate the diagnostic value of transvaginal color doppler ultrasonography in endometrial benign and malignant lesions. Methods 136 cases of transvaginal color doppler ultrasonography of endometrial benign and malignant lesions which were proved by operation were analyzed retrospectively, and they were compared with pathology results. Results The transvaginal color Doppler ultrasound of submucous myoma, endometrial polyps, carcinoma of endometrial is consistent with Pathological Diagnosis in the higher rate, they were 87.5%, 90. 0% and 80.0%. To observe and measure the echo of Endometrial thickness and shape by transvaginat color Doppler ultrasound, it is helpful for clinical to diagnosis and treat in atrophic uveitis, endometrial hyperplasia simple, and it can avoid unnecessary consultation scratch. The existence of the low resistivity diastolic blood of Intimal thickening is important to diagnosis endometrial carcinoma, and the assessment of myometrial invasion of endometrial carcinoma which is according to the hypoechoic area between endometrial and myometrial interrupt or not is great significance to surgery and prognosis. Conclusions There is a higher value in differential diagnosis of endometrial benign and malignant lesions by transvaginal color doppler ultrasonography, and some of the overlapping of sonography needs to be comprehensive analysis to improve the diagnostic accuracy rate.
